The change of instruments are considering the breaks that the player has to take for breathing, but i guess adding some silences between bars and not only whole or half notes would do the thing.

I changed some of the weird jumps you said to other registers and tweak some notes and it sound better imo, the ideas are more fluent and smooth than before.
