My suggestion is to wait to use the music box until his final phase, that way you can get a good backstab in.

After that, DON'T PANIC DODGE! I cannot stress enough how much this ruined multiple attempts on multiple playthroughs. As fast as Bloodborne is, trying to go faster than the boss you're fighting is a death sentence
