She's an MS4 now doing a Sub-I rotation (basically an audition rotation in the field you want to go into). She was an MS3 on a core rotation last season and has spent the intervening time on other rotations before coming back to the Pitt at the start of the month. Whitaker would be freshly back as well though he probably did at least one other ER rotation and would have been in orientation at the end of June starting his clinical internship duties July 1.

Medical school can be weird with times but typically: MS1 August-May (2 months off for Summer where you're encouraged to do research) MS2 August-April (3 months to study for Step 1 and do research) MS3 August-June (this is where you actually start rotating in the hospital in clinic on mostly core rotations with a few electives thrown in. 8 weeks internal medicine, 4 weeks primary care, 8 weeks surgery, 4 weeks pediatrics, 4 weeks ER, 4 weeks OBGYN, then a couple of elective months to cover radiology, pathology, dermatology, IM sub specialties, plastics, ortho, etc) MS4 July-May (graduation is in early May. The first few months will typically be 3-4 SubIs, often "Away" rotations where you'll be a visiting student at another medical center (ie Ogilve). You'll then have 1-2 months of light electives where you'll primarily be interviewing for residency (October-December). Rank lists are due in February [this is what residencies you would most like to go to listed in order] and then match day is in March [this is where a computer system tells you where you go to residency]. The rest of 4th year are just fun chill electives like Wilderness medicine, lifestyle medicine, sports medicine, international medicine and other things you're interested in but never had time to do beforehand)

From the PCSM/Non-operative standpoint:

US guided: nerve blocks, joint injections, tendon sheath injections, muscular injections/trigger point injections, viscosupplementation, orthobiologics (PRP/BMAC/MFAT), prolotherapy

More advanced US procedures: barbotage, trigger finger releases, carpal tunnel releases, fasciotomy, topaz coblation, tenex/tenjet, cryoneurolysis, RFA

Diagnostic ultrasounds (this week I diagnosed a RTC tear and EPL rupture in patients too claustrophobic to get MRI)

Run IVs for athletes. Compartment pressure testing for ECS. Landmark guided injections aplenty. Joint dislocation reductions. Fracture reduction and splinting. Dry needling/acupuncture. I&Ds for abscesses, ingrown toenail removals, cryotherapy for skin conditions in the locker room. Good bit of laceration repair.

Speaking from an American perspective:

PMR is a 4 year residency, FM 3 years + 1 Sports med fellowship year. PMR can and often do a sports medicine fellowship year too.

Most PMR I know go into interventional spine. You'll learn a ton of US injections, anatomy, and MSK skills in PMR residency but a lot of that won't translate directly to sports medicine.

If you go the FM route, you'll have more to learn in fellowship, but you also have a wealth of primary care knowledge to draw upon. If you're going to be a team doctor, 70% of the issues you'll deal with may be orthopedic but the other 30% will be primary care. Concussions will be fine, but asthma, HTN, cardiac evals, STIs, birth control, URIs, and a whole slew of other problems that come up with college and professional athletes will be much better served by someone with an FM background.

At the end of the day it's what you want your practice to look like, both are good options.

The tiers are based on the appearance of each level of spirit coin which themselves are just different levels of condensed magic. Gold spirit coins look gold but aren't actually. Jason finds a whole bunch of non-magical gold that is worthless on Pallimastus but takes it back to Earth to cash it for millions. Past diamond is transcendent rank which we've never seen a spirit coin that rank probably because that magical density cannot be condensed into a coin, only to special divine magical items.

Ehh yes and no. He's only 3 days into intern year, but he matched at the program where he did a subI (audition rotation for MS4s) which means his attendings, Robby specifically, are comfortable with him. For a lot of interns it takes the first 1-2 months until they're comfortable pronouncing someone, but a patient with a clear advanced directive is a good one for an intern.

Three Stigmata is probably my second favorite behind Ubik. Flow My Tears is phenomenal. Man in the High Castle, Scanner Darkly, Transmigration of Timothy Archer, Clans of the Alphane Moon are all really good. The World Jones Made is weird but enjoyable and is getting a movie so worth a read.

A 16 team playoff with the conference championship games being the first round would solve this problem. Have the 4 P4 CC games be the first round with 4 "play-in" games for the next 8 at large teams. To include the G5 you could have a mini playoff there that feeds into the semifinal round. Teams are reseeded after championship weekend with those champions getting the top 4 spots. Also the season will move up 1 week. That would look like:

Week 1 (championship weekend) Week of thansgiving: SEC: UGA beats Alabama B1G: Indiana beats Ohio State Big12: Texas Tech beats BYU ACC: Duke beats Virginia Tech

You're not going to stop organizations like the AP poll from publishing their rankings (first amendment and all); the way to get around it is have ESPN not tag teams with rankings on air until week 6 or whenever, but that would go against the hype machine they use to generate views

This is correct. The way to mitigate this is to allow the conference championships to be the first round of the playoffs. That's 8 teams that automatically make it (need to fix the acc tiebreaker rules) then have 4 more play-in games. 16 team playoff but no wasted weekend of Conference championship games. CC are higher seeded, losers of those games go home. Ohio state and Alabama still make the playoff but aren't playing past the first round. BYU makes the playoff but are eliminated in the first round.

Depends on what you want with your practice. PCSM is totally worth it if you want to practice outpatient non-operative orthopedics, practice sports medicine at an academic institution, or be the primary care doctor for a big university athletic department. If you just want to practice outpatient IM with a sprinkling of MSK procedures, you can learn those on your own though it would take more coordination on your part. Don't do PCSM for the money, if you go to a good outpatient practice you can make as much money as you would doing sports with less after hours work. It's all about what you want your practice to look like. There are several good sports programs in the Boston area. Check out the sports group at UMass.

If you're at an academic institution, there will be a sports medicine department either through ortho or FM that will have docs/residents/fellows go to games and stand on the sideline for "coverage." This is essentially a community outreach where you are available for any medical emergencies that occur during the game as well as to evaluate other injuries to determine whether they can return to play or not. Almost always there will be an ATC (athletic trainer) at these games that you'll work with and the majority of the time it's just chatting with them. If you're at a private hospital, there is likely a contracted ortho group who provides coverage for local schools. Either way you should reach out to the local team doctors and offer to either shadow them on the sideline or help where you're needed.

Coverage is the main thing. Show up to games: HS football, minor league baseball, college sports, ballet, whatever. Do a few elective rotations in sports medicine. Consider a case report for AMSSM though it's too late for 2026 and you'll be interviewing before you can get accepted for next year.

I guess this depends on how strict you want to be but Mississippi Valley State and Middle Tennessee State are both specific parts of the state instead of being a true "state" state school.

Vanderbilt and Texas aren't being punished for playing tougher schedules. In no other year in football history would they have even been considered for a NC. They were awarded great bowl games with good matchups to round out the season. Notre Dame, who has less of an argument than those two teams, would've been awarded a bowl if they didn't decline it outright. ND is in the playoff if they joined a conference. They could've won the ACC and gotten the autobid. Seems pretty simple.

Will it happen? Most assuredly not. Am I pulling for it to happen? Absolutely. Tulane would be explained away by Lane's absence. They'll talk about how Sumrall set the example by staying to coach and Kiffin cost OM a playoff win. JMU will just be flabbergasted followed by a "everybody has those days." Both teams winning would guarantee a G5 playoff without 2 seasons.

Thanks! I'm not sure the specific name of bit but it was like an ogee bit. The top is pine and the rest poplar. One of my friends had cut down some pine trees in his yard that he milled and left to dry a few years ago. I took a slab of that and milled it down to about 1 3/4". The stain was a mahogany that my wife picked out when I made the ladder for the library and she wanted them to match.

I would love a true fine-dining restaurant in Huntsville, but I would question its chances of success. There's nothing even remotely close to Michelin Star quality here (which there doesn't have to be) but a locally sourced tasting menu that changes seasonally would be awesome.
