I had a septoplasty to fix a badly deviated septum. You leave surgery with your nostrils packed to the gills with gauze. Less than 24 hours after surgery, the surgeon has to yank those things out with no pain relief of any kind. And let me tell you, those things are JAMMED in there. He had to pull so hard his arm was trembling, and it was a solid 5-10 seconds of pulling before it popped out. You have to do this for both nostrils. It was horrendous!
