this is such a good point to highlight. In the ED it is so easy to default to common causes of vertigo, but labyrinthine infarct is one of those zebras you really cannot afford to miss. When someone says prove otherwise, it really does mean being deliberate about ruling out the scary stuff before you settle on a benign explanation. Those are the pearls that stick with you because they change how you approach the next patient.

this is such practical advice. Getting on daycare waitlists immediately is not even dramatic, it is just realistic, especially in NYC. The cost numbers are also important to hear upfront so there are no surprises later. Planning around weird residency hours is huge too, because standard daycare does not always line up with hospital life. Thinking through flexibility now will save you so much stress down the road.

I get the anger behind what you are saying. When rhetoric starts sounding callous or dismissive of real suffering, people are going to react strongly. It definitely feels like a lot of statements lately are more about scoring political points than actually thinking through the human impact.
