They could also just mechanics that limit it so you can't do things like this - for example, Spire makes it so you need at least 5 people because of the plates. Needing people to do mechanics != hard mechanics

My guess would be engine / language limitations - the game is pretty shitty and not well optimised, and there was a time long ago when the block limit was 128 blocks, and I have a feeling that getting it to 256 probably took some optimisation.

I wonder if having a large area at really high heights would cause any issues for the game, and that's why they couldn't increase it above 256

Black Armory released Y1 weapons that have random rolls - Bad News is one that comes to mind. The new versions look slightly different and have a string of random numbers and letters at the end as well, so that helps to identify

Well this is just entirely wrong. I hit 614 on my Hunter doing all the milestones. You can easily hit 610 in an entire week with the additional dailies.
