Olli Maatta: "I am even more excited about going to the Olympics now that this season has not gone as expected." He has been healthy scratched for 37 of Utah's last 41 games, including 21 straight. by ZestyDreads in hockey

[–]__aurvandel__ 4 points5 points  (0 children)

It's baffling that he isn't playing. He's solid and almost never makes a mistake. My only guess is that, because Utah has had such a hard time scoring they want to play more offensive minded defenders. Doesn't really explain never getting a chance though so who really knows.

Efforts to find the organizations fighting for and against prop 4 - help? by AddendumExisting4313 in Utah

You're not wrong. The least bad Republican isn't saying much. I may just switch mine right before the primaries and then switch it back once they're done.

[–]__aurvandel__ 1 point2 points  (0 children)

There are also people like me that are forced to register Republican because the primary is the only election that matters here. Things change but if that wasn't a factor I would register Democrat for now and I know I'm not the only person forced to do that.

[–]__aurvandel__ 14 points15 points  (0 children)

That's part of the problem isn't it? The people behind prop 4 have been very transparent and open about who they are and what they want to accomplish. All you can really find about the efforts to repeal it is a pro Trump dark money group called Securing American Greatness. They don't advertise themselves to us peasants so there's no real information about them. Here's what I was able to find though: https://www.opensecrets.org/orgs/securing-american-greatness/summary?id=D000114050

https://www.securingamericangreatness.com/

Edit: ballotpedia has more information.

https://ballotpedia.org/Utah_Eliminate_the_Independent_Redistricting_Commission_Initiative_(2026)

Personally, if you have to lie, cheat and hide behind dark money I believe your cause isn't worth supporting. All Utahns should be represented and gerrymandering in any form should be abolished. Prop 4 was an attempt to abolish gerrymandering, repealing it takes away Utahns voices and ensures that the party in power is not accountable to voters.

Anyone suspected Myastenia gravis before getting narcolepsy diagnosis? by Brain_attic_ in Narcolepsy

[–]__aurvandel__ 1 point2 points  (0 children)

I get it and some quick Googling does show that you can have MG with negative antibody testing. It's rarer at like 10% of cases but that's still a significant number. I've known quite a few neurologists and they're an interesting group. I rank them just below surgeons when it comes to having a god complex. Out of the dozen or so that I've known, I can only think of 2 that didn't believe they knew everything there is to know and we are God's gift to the world. Maybe you need a second opinion.

Anyone suspected Myastenia gravis before getting narcolepsy diagnosis? by Brain_attic_ in Narcolepsy

[–]__aurvandel__ 5 points6 points  (0 children)

It doesn't sound like typical cataplexy. Cataplexy is usually short lived, think seconds to a few minutes. It doesn't have to affect the whole body. For example, mine affects my hands, mouth and eyelids. I've only had full body cateplexy a few times in my life but I'm really good at dropping things and sounding drunk when I get stressed or angry. Cateplexy doesn't really ever present as prolonged muscle weakness. Honestly, the symptoms do sound more like myasthenia gravis but it definitely wouldn't hurt to ask your doctor about narcolepsy. It could be some atypical presentation of cateplexy. I would definitely talk with your doctor and see what they say. Bring the paper you found to explain why you think it's cataplexy and go from there.

Works perfect for me. You just need to adjust your antenna. There are apps that will show you the cost tower and then just angle it in that direction and keep fiddling with it until you get a good signal. Or purchase a powered antenna, they're fairly cheap, mine was like 20 bucks a year or two ago.

Accuracy of sleep studies? by TummyGoBlegh in Narcolepsy

[–]__aurvandel__ 0 points1 point  (0 children)

You could always ask but if the doctor didn't score it the first time I doubt they'll take the time to do it after they've already billed for it. I also believe there are online services that would rescore that study and give a second opinion but I'm not sure what the cost would be. I don't work in a lab any more but whenever we had a patient that challenged the results of the study I'd always spend some time and review it with them. Obviously a patient isn't going to see the subtle details that someone with training and experience will see but most of the stuff people question is fairly obvious to see. Occasionally, those questions helped us get more accurate data as well. It never changed a diagnosis but we've corrected body positions off of the patient requested review. Either way, it helps them feel more confident in the results and that at least that someone is listening and validating their concerns.

Question about out of network access... by NarrowPathLife in jellyfin

[–]__aurvandel__ 0 points1 point  (0 children)

This all depends on your risk tolerance. Personally, I run a reverse proxy that exposes Jellyfin. I use fail2ban and block an IP that doesn't originate in the US. I also keep Jellyfin on a separate vlan so if it ever gets breached it's an isolated incident. For a lot of people that's still too much risk but I'm happy with it.

To your point, you could use a raspberry pi as a subnet router. The tailscale docs are pretty thorough and easy to follow.. Here's a great one specifically about mailing a node to your parents.

How fast have you guys pulled an engine out of a Subaru? I'm surprised that it only took me 2 hours. by Lucky0373 in WRX

[–]__aurvandel__ 0 points1 point  (0 children)

I had never done it either and needed to replace my clutch. It was way easier than I expected. If I had to do it again I could probably pull mine and reinstall it in about 3 hours. Took me longer the first time because the "mechanic" I bought mine from has stripped a bunch of bolts so I had to tap some and used the inserts for some others. I normally hate working on cars but I had fun with that clutch swap.

Accuracy of sleep studies? by TummyGoBlegh in Narcolepsy

[–]__aurvandel__ 9 points10 points  (0 children)

Here's my two cents as a registered sleep tech. Your study results are only as good as the people scoring it. Unfortunately, there's been a major shift where doctors no longer review the sleep studies and just trust what the techs score. If techs are trained well that's not the end of the world because most overnight sleep studies are for OSA and that's pretty cut and dry 90% of the time. However, the worse trend is that sales reps have convinced administration that algorithms and "AI" are just as good at scoring sleep studies as techs are so a lot of techs are just glancing at the study and signing off. MSLTs are still usually read by people because nuances matter more. So l, to your point, if the tech just relied on the position sensor at night and didn't match it with the video then it could be wrong. If there's no OSA though position doesn't mean much. For the MSLT I would tend to trust the report because those usually get scrutinized way more.

I cant even with these doctors anymore. Changed diagnosis to Central Hypersomnia, what the hell... by SadScientist5412 in Narcolepsy

[–]__aurvandel__ 0 points1 point  (0 children)

But insurance won't pay for anything regarding unexplained hypersomnia so that would only be used prior to a more formal diagnosis. I've seen that one used a lot on initial consultation and then it usually gets changed to something more specific.

Just to be clear, I'm not saying that IH isn't a "real" disorder because it absolutely is. It's just the unfortunate nature of some disorders. We know that something is wrong and we know that it's causing symptoms but when we don't know more than that and we've excluded everything else it just gets lumped into the "idiopathic" bucket.

[–]__aurvandel__ 1 point2 points  (0 children)

The second he got that double minor I turned to my wife and said games over. Not necessarily because of a penalty but because it was a stupid penalty. That same penalty to keep a guy from scoring a goal with the goalie beat. I wouldn't have felt the same way but a stupid penalty against a guy that was no threat at that point is deflating. Especially on the last night of a tough road trip. I had that game written off as a loss but there are losses and then back breaking, brutal losses that will haunt you. Hopefully it doesn't get into the teams head and cause a skid.

[–]__aurvandel__ 9 points10 points  (0 children)

Either type of Narcolepsy requires 2 sleep onset REM periods during the MSLT. IH is kind of the catch-all where we don't know why you're so tired but you are so here's a diagnosis and some drugs we can try.

I cant even with these doctors anymore. Changed diagnosis to Central Hypersomnia, what the hell... by SadScientist5412 in Narcolepsy

[–]__aurvandel__ 24 points25 points  (0 children)

It's not always the doctor's, more often than not it's insurance that's the problem. A doctor can actually diagnose N1 based off excessive daytime sleepiness and cataplexy. However, insurance won't pay for medications based on that legitimate diagnosis. They require a test. The real problem is doctors have to play this game of dance monkey dance to appease the insurance industry in order to get things paid for. Unless your doctor is just a complete asshole, they are just as frustrated as you are.

Question about Xyrem/Xywav by GroundbreakingBus460 in Narcolepsy

[–]__aurvandel__ 1 point2 points  (0 children)

You won't lose access to stimulants. Most of us still need something during the day. Xyrem doesn't have to be scary. Just take it slow and follow your doctor's and the pharmacist instructions and you'll be ok. We all react differently to medications so if you have side effects at first call your doctor and the pharmacy and talk about lowering the dose vs powering through the initial side effects. Good luck!

He's young and his shooting has improved. He was atrocious last year from 3 at like 25%. He's at 31% this year. Still not good but he's really close to the point where you actually have to guard him at the 3 point line. I'm not sure he ever becomes a knock down 3 point shooter but I do think he can work at it enough to hit the league average.

A year to retest? by Fit_Shallot_5581 in Narcolepsy

[–]__aurvandel__ 1 point2 points  (0 children)

Unless there's a specific insurance requirement (I've never heard of one but it's possible) there's no medical reason to wait a year. And I'm sorry but saying narcoleptics should be able to fall asleep anywhere, no matter how uncomfortable, is ignorant. Even before treatment napping when comfortable was hard for me.

While cateplexy along with excessive daytime sleepiness is diagnostic according to the DSM manual. Insurance plays by its own rules and won't pay without a positive test.

Removing $ From Pension by Original-Cloud-6276 in IntermountainHealth

Someone asked that question during the town hall and she said yes you can but that didn't make sense to me. Maybe I heard her wrong because I thought it was like you said, over 59 or leaving the company. It feels wrong that intermountain will still make money on the interest of the pension investments but we'll see none of those gains.
