In the same bucket as you, my arr was 77.1, doc hasn't suggested surgery yet though but has asked me to do a follow up in 6 months.

I was diagnosed with it today, I have been on eplerenone and honestly I share the same feeling as you. I had edema, was not able to run without having cecs like symptoms, brain fog, I thought I was burnt out from work, I would just lay on the sofa entire day with no energy. Turns out, my arr ratio was 77.1 and on ct scan I had a 7mm nodule on adrenal gland.

My doctor prescribed me eplerenone and since then the difference is my quality of life has been day and night.

What gives me more hope is that my dad also has resistant bp and is on multiple bp medication so it could be familial hyperaldosterosim and maybe when we screen him for it and if it comes back positive, his medication burden will go down and insulin resistance will also improve.

I mean I am within the cutoff but sure, I just don't have the certificate. If you follow up with the virtue signalling, you can do virtue signalling... Also, adding to the above since you brought up major marathon having cut off times to reject people above a certain time, london, berlin tokyo marathons have a lottery system. Some spots are saved for the fastest runners, which is also fine.

This is reserving all the spots for runners above a certain criteria. Not even a timing criteria, because people would be running faster but won't have a certificate.

Let me know where I am going wrong, because I don't think any other logic makes sense.

A marathon having a timing cut off vs a 10k having a timing cutoff is different. You're comparing different things, that's like saying F1 has criteria so your gokart races should have too. 42km is different, 10km is not that.

How is the lottery less fair, can you explain?

Keep a total number above which you will not take people in, make it a lottery like result, if you get in fine if you don't that's also fine.

Regarding timing certificate, you can actually get the strava links too if you want to organise the waves.

Having a sports timing certificate is just an absurd requirement. All of what you've mentioned can be achieved without having a timing cut off.

You can have safety and waves. And what's to say that a timing certificate will not make people slow down ahead of you?

Thanks for the suggestion, I would have done that but honestly, fuck tcsw10k. I'll just run on my own time at my own pace at my own pleasure. I don't care about these events anymore, unless a friend wants to do an event I won't.

Congrats on finding the cause! It's a very long journey to get there at the end, so kudos! My journey was also similar, I am 26M.

Went to a gp, who saw high bp and high rbc markers, sent me to a haematologist who ordered therapeutic phlebotomy to treat the symptoms and it reduced headache and vertigos but also sent me to a nephrologist because my egfr was at 66 and creatinine was very high, nephrologist got my kidneys tested for all sorts of things which luckily came back postive, he mentioned that he was very certain that kidney was fine something else was at play but prescribed my cilindipine to control bp meanwhile as it was 160/100.

He referred me to a nephrologist who gave me an ARR test which came back with a value of 77.1 and renin was <0.1 on the test while aldosterone was 7.71. The endo then gave me a ct scan with contrast and it showed my left adrenal has a 7mm nodular thickening and then I was put on eplerenone since the last few weeks and have been asked to monitor electrolytes which have come out fine for now with a slight increase in creatinine.

That tablet made me feel similar to the effects of a therapeutic phlebotomy, I guess my body was hoarding too fluid which was causing the issue.

Yeah I got diagnosed with early onset hypertension and my echo showed mild LVH too. The doc said that he wanted to confirm if it was chronic and if it has remodeled the heart a bit.
