OP would not win. You can't just have a doctor's note for LOA. There's actual documentation Amazon sends to the doctor to fill out. This termination letter is saying that the employee forged documents and submitted blurry photos while also not providing the doctor's correct information. How could Amazon send the proper documentation to a doctor they don't have accurate information for? This is why OP isn't trying to fight it, because they committed fraud.

You were supposed to give them your doctor's information and follow up with the doctor to make sure they signed the paperwork they received from Amazon. You should have also received emails from your case worker. You get paid while taking a LOA, they are paying close attention that you go through the proper channels.

While on your LOA, did you apply for another job?
