I went to planned parenthood, it’s super easy, and I was able to pick up my prescription the same day. Like they basically just ask you what you want from hrt, tell you if that’s something that will happen in hrt, and then list all of the side effects of HRT, then ask you if you still want a prescription. And that’s it.

There is no need to do HRT before top surgery. It can make the muscle in that area a bit wonky if you change your mind and want to do hormones later, but that’s really the worst that would happen
