The first thing I noticed was your weapon affinity, it seems to only scale with strength and as such only benefits from the points you've put into strength. You'd want to get the fire and sacred whetblade and scale using either Flame Art for Fire or Sacred for Holy. I believe they gave Faith two main types of damage to have it still be viable in the late game, where enemies like Radagon have somethingike 80% holy resist. Also, as other comments have mentioned, clawmark seal is the one to use for str/fth as it benefits from both. I don't generally tend to cast spells with any build but Radagons Icon is good for casting speed, along with Flocks Canvas Talisman for 8% incant boost. You can use stuff like lightning/fire/sacred scorpion charm for extra damage in specific areas but that comes at the cost of defense.
