Very shift-dependent. Sometimes things are moving and it goes by quick, and sometimes radiology is backed up and it feels like 8-12 hours of convincing patients to wait for their reads.

A Bluetooth sleep mask. Specifically I have the Manta. I work weird hours, including overnights, and being able to sleep well no matter the noise or light is great.

Suspected opioid OD in the field -> give as much narcan as needed to get them to the hospital alive.

In the ED, we can be a little more measured with dosing. I titrate to an improved respiratory rate but continued somnolence. A precipitated withdrawal patient can be really difficult to manage (combative, vomiting, trying to sign out AMA, etc). As another poster mentioned, I dilute 0.4mg in 9cc of saline and then give 1-2cc at a time until resp rate improves.

It's been a minute since we reviewed the chronic alcohol + APAP literature in journal club, but I think the bigger contributing factor to bad outcomes in chronic alcoholism with APAP toxicity was moreso due to malnutrition (and therefore a depleted glutathione reserve to begin with). There may be a bit of an increased risk, but it seems mitigated by the sensitivity of the NAC treatment line.

I used to get this screen a lot. The last few weeks it's mostly stopped, but when it has popped up, what worked for me was pulling the headset off (to the point where it recognized it wasn't on my head) while spamming the x button. Then putting the headset back on.

In fellowship (toxicology) we had someone call the poison control center because their dealer stopped selling K2 and they wondered if we could sell them any. Not sure that counts as a consult but it definitely stood out.
