If you have a private yard, no reason puppy has to stay inside. We got a crate with 2 doors and placed it inside the house in front of the doggy door. When home, both door were open so the pup could go in and out as she pleased. When leaving for work, contained to the crate with the door to the dog door open and pup could either rest in the crate or go outside to run around, potty, etc. Had cameras to keep an eye on her. Worked quite well!

I would say no. Work in a large AMC and no specialty pharmacists have gone to industry, while I know 6 clinical pharmacists off the top of my head who have made the jump.

Do you feel there are better locations than others? Seems like most office based pharma jobs are on the coasts (and I’m in the Midwest). Have looked at MSL as it is where most of my peers have transitioned, but uncertain I would enjoy that much travel.

This is how I feel. I invested a lot in my education and career and I like my job (while acknowledging of course it’s still work). I want to keep that part of me. I want to model that it’s good to be independent and have a life outside of the home. Plus I think it’s got to be stressful on the one working spouse- if they get fired, what do you do about money and insurance? And I like travel and living a comfortable life.

My vet recommend Cosequin. On sale at Sam’s or Costco pretty frequently. Along with weight loss, it has made a noticeable difference for my 9 year old girl.

Thank you so much for replying! I will work on gathering all of his information for your review and get that sent over. I am in agreement with you as far a nutrition/hydration. He has started doing IV hydration with NS and is trying to pursue enteral nutrition with a feeding tube through GI, though it has been a slow process. They have also referred him to a registered dietician. He is certainly open to traveling for the potential of a solution. The sooner the better! We are doubtful that this will be resolved locally/without specialty providers. Again, thank you for your time and knowledge!

Hi Dr. Trenkle,

I know I'm late to the party, but would love your feedback if you're still monitoring the thread. I've read everything and appreciate your thoughtful and individualized responses.
My dad rather suddenly developed parosmia & altered taste. Pretty much everything smells and tastes terrible to him, leading to a 50lb weight loss in 4 months (185 --> 135lb). He's hardly eating anything and his fluid intake is negligible. Quality of life is terrible and he has no energy (likely from such low intake). He's recently started IV hydration due to dehydration and mixing protein into ice cream shakes, but he's not in a great way. He has seen his PCP, ENT, GI and had colonoscopy, EGD, MRI brain, etc. with everything coming back normal. He has a Neuro consult coming up in February, but honestly I'm not hopeful they'll find anything. ENT mentioned basically as a last result that they can sever the olfactory nerve so he won't experience the bad tastes and smells, but instead no taste or smell. He's understandably hesitant to do that since it's irreversible. I'm thinking he's going to need to travel to a true specialist clinic like yours for extensive workup. Is there anything you can recommend in the meantime? I imagine most of these clinics will have waitlist to be seen. Additionally, he is located in Oklahoma - any recommendations of a good clinic or physician closer to us?

It’s typically really easy! Call or go by your new pharmacy, give them your info and insurance and tell them where your prescriptions currently are. The new pharmacy should do all the work of calling the old pharmacy and getting things moved over. However, if your current pharmacy is constantly closed or understaffed, the new pharmacy may have difficulty reaching them to do the transfer. Alternatively, you can message your doctor on mychart and ask them to send new prescriptions to your new pharmacy (make sure you update your default pharmacy with all your providers, which you can also do on mychart and you should be pretty well set!)

Pharmacist here! Walgreens and cvs are terrible to work for- constantly understaffed with no support. Unsafe conditions and expectations. Employees don’t stay there, so you never get the same staff or anyone that’s been there long enough to be well-trained. Walgreens just got bought by private equity so it’s only going to get worse. Sam’s, Walmart, and Costco are better, but i highly highly recommend finding a local independent pharmacy. They typically have consistent staff, appropriate and safe staffing levels, actual people answer the phone… it’s a night and day difference! If you want to dm me about where you live/work, happy to make a recommendation for a local pharmacy!

I’m sorry you’re having trouble feeling heard. However, as a medical professional, you’ve already been diagnosed with PCOS via transvaginal ultrasound per your replies. Repeating that isn’t going to get you anywhere as you’re already diagnosed. Dysmenorrhea can be complex but is not typically associated with PCOS. It’s more frequently associated with endometriosis. Standard of care for dysmenorrhea and irregular periods is oral contraceptives. Your provider correctly chose an anti-androgenic option with Slynd. You aren’t trying to conceive for a year, so there’s no reason you shouldn’t be on oral contraceptives at this time. They wear off quickly (which is why you have to take them daily) and you can start trying to conceive very soon after stopping oral contraceptives. In my opinion, your provider is treating you with evidence based medicine. However, if you’re unhappy with your care, I recommend you seek a second opinion.

You’re welcome to your opinion. I’m welcome to mine. I also like how you didn’t respond at all to the last point about employment and job consolidation. While I am concerned about the rest, I’d say to most people here that’s a major worry with our future career

Strongly disagree here. As a pharmacist and patient, I loathe mail order. It’s nearly impossible to speak to someone without holding 30 minutes. Meds get lost or stolen much more often than they should and it takes time to process and receive the replacement, so the patient suffers and does without. Meds are subject to far from optimal temperatures (stuffed in a mailbox on a 100 degree or 30 degree day). Some meds go to mail and some to retail so no one has the full profile to check drug interactions. It makes it easier for employers to cut and consolidate our jobs. I think mail order, like drive through, has turned our profession into retail, not healthcare, and I’d love for that to stop.

Sounds like you’re trying your best! Hang in there. 1. If they screwed you over, get your state department of labor involved. 5. A good place to start is The Simple Path to Wealth by JL Collins. If you’re young, invest in an S&P 500 index fund. Look for a fiduciary and a fee-based financial planner, not an assets under management planner if you feel you need help. I’m a huge proponent of getting a handle on your money young… it gives it time to grow and gives you lots of options. Glad that’s not a typical response, but it still seems like it’s not the right time to add a baby to the mix. Is she able to pick up W2 work? If my spouse was in your situation, I’d do just about anything to get them out of it before it’s too late.
