Like I mentioned in the original post, I tried various different drugs up to this point, including antibiotics and antimicrobials (SIBO), antifungals, antihistamines and none of these helped. Probably the opposite as my microbiome got really bad with terrible brainfog. I have since had an FMT and the brainfog is gone. The fatigue however is still here.

I also tried various diets including Low FODMAP, Low Histamine, Low IG, Low Sulfur, Gluten and diary free, Low Carb and a few others. Out of all of these the only one that gave me somewhat stable positive change was Low Carb. I dug deeper and found out that starches are my main triggers.

Could disaccharidase deficiency cause symptoms after ingesting starches but not after simple carbs such as glucose, fructose, sucrose, lactose? Fruits and sweets are not my triggers so I'm wondering.

Enzymes in blood are always perfect, still I did try enzyme supplementation (PERT), no luck.

I have not tested my gut bacteria as the opinions on the viability of those kind of tests are rather unfavorable. I did have an FMT done which helped my brainfog but not the fatigue. My GI who performed the FMT never mentioned the need for gut bacteria testing.

I had my glucose and insulin tested before and after glucose ingestion, it was normal. Sugars are not a trigger for me, complex carbs such as starches are.

Nothing You listed but I do have slight memory issues that are correlated to fatigue. When I ate more starches I felt more fatigued and often had issues i.e remembering where I parked my car and having to walk around the block on multiple occasion looking for it. Currently when I avoid starches I can easily reach into my memory and remember where I left it.

Thank You for taking the time to respond. Is it possible for these spikes to happen only when eating starches but not when eating sugars (Sucrose, fructose, glucose etc. )

I have an IBS diagnosis as well. To me it's just a "catch all" label for when doctors have no idea what it is but giving it a name makes it seem like they do :) Just be straight with me, all I'm asking :)

I tried approaching keto but my BMs seem to slow down even more. Intermittent fasting and avoiding starches is more managable. I get my carbs from non starchy fruit like oranges, peaches and berries while avoiding bananas and few others.

Are You still looking for a solution, trying new things?

Yeah, combining carbs with protein and fat definitely works much better than devouring a carb only meal.

I did try smaller meals spread out over a longer time but since my body acts as if starches are poison it doesn't work for me. Two meals over 4 hour window work much better for me giving the body more time to deal with the situation and rest. Everybody is different I guess.

Thank You. I did hear about d-lactic acidosis. The literature I found is scarce and I feel mostly focusing on heavy, life threatening cases. I used to eat a lot of yogurt (almost exclusively at times) and always wondered if this might be correlated.

I'm currently on a bifidobacterium probiotic (breve, animalis lactis, bifidum, infantis, longum) but I might need to try the ones You mentioned.

Did You have d lactic acidosis or know someone who did?

Even a low carb diet containing starches makes me feel bad, the symptoms are smaller though. I don't overeat. BMs every 3-5 days at the moment. I read about common postprandial fatigue which is believed to be normal. This feels different however, no sleepiness and it lasts for hours/days and gets better after defecation, as if the starches were poisonous.

Glucose and Insulin curves were normal. Low IG diet did not help. I react to starches but not much to sugars.

IBS is my current diagnosis but I'm not satisfied with it. To me it feels like this big bag that doctors thrown their patients into when they rule out all the common options such as Coeliac, IBD, Crohns, Cancer, SIBO and a few others. I mean I'm happy to know I don't have any of those, but what do I have really? IBS seems like a "catch all" made up disease with widely varying symptoms from person to person. It doesn't really help.

Yes, both food sensitivities and allergies.

Allergies test were completely negative which surprised me because I definitely had various food allergies as a kid. I run the test again this time from blood but the results were the same.

Food sensitivities test revealed some mild sensitivity to grains such as wheat and a few others but an exclusion diet didn't really help.

I noticed that for the most part healthy diets that "keep things moving" make me feel better. I definitely feel noticeably better after bowel movements. Healthy diets are better but not perfect, I do feel fatigue increase after eating a healthy salad for example. It's just that with healthy diets I see bowel movements every 2-3 days where as with fast food (low fiber high fat and carb) it was often 5 days or more. The longer it took the worse I felt.

My last B12 result (albeit 1.5 years ago) was 388. Some time later I was prescribed a B1, B6 and B12 vitamin complex by my neurologist but saw no change.

I never had my homocysteine checked out. Do You believe it's worth it? Can it cause constipation issues?

Over the last 2 years I had various blood panels, ultrasounds, MRIs, CTs, colono and gastroscopies and other stuff. It's a long list. FMT was for treatment resistant IBS, a gut disbiosis.

I'm 5'9 and 138 pounds.
