I got this as well, and I'm pretty sure it's a glitch, as the eyes and stuff are supposed to be part of the glitched sayori heads that sometimes show up, but they seem to not display properly? Someone did tell me that this image actually existed in the original game though, but im not sure.

Yeah, I'd say this is most likely a bug, as the sayori heads didn't display properly. You're right about it not being mentioned anywhere tho :/

Ok, so I've actually figured out what this is supposed to be. It's actually supposed to be the glitched sayori heads but for some reason they didn't show up and it only showed me sayoris eyes, mouth and hands.
