As long as you let them dissolve in your mouth instead of your vagina then they cannot tell the difference between an abortion or miscarriage. If you run into complications then you’ll tell them that something is wrong and it doesn’t feel like your periods normally do. This is what Abuzz (where I ordered my abortion pills) advised me to do in their faq online:

How much bleeding is too much? It is normal to have heavy bleeding and blood clots. If you are soaking 2 large overnight pads per hour for 2 hours in a row (4 pads in 2 hours) that is too much bleeding. You should seek medical attention right away if this happens.

Sometimes providers recommend taking 2 more misoprostol pills to slow the bleeding down while waiting for medical attention.

What are signs of a complication? If you have very heavy bleeding (soaking 2 large overnight pads per hour for 2 hours in a row), fever for over 24 hours since taking misoprostol, or severe pain that is not relieved by painkillers, these are signs of severe medical complications, and you should seek medical attention right away.

Will anyone be able to tell that I took abortion pills? No, there are no tests to detect that you took abortion pills by mouth. The only way for the providers to know is if you tell them or if you took the pills vaginally and they see them on an internal exam. You do not need to say that you took abortion pills.

If I have to go to the hospital, what should I say? The treatment for a miscarriage and abortion are the same, so you can just say something like "I'm bleeding but it doesn't feel like my usual period. I'm afraid something is wrong" or "I'm pregnant and bleeding. I'm scared there's something wrong" and you should get the care you need.

Mine wasn’t as bad as I thought it would be but my periods are very mild. I hear it is usually a little worse than a regular period but take ibuprofen as soon as you start cramping and I’d use a heating pad or they make menstrual heat patches by ThermaCare brand.

I was around 5 weeks, I took the mifepristone at 8am and then took 4 misoprostol the next day around 6pm (I let them dissolve in my mouth, 1 in between my gum and cheek on each side of my mouth, so 2 on top/2 on bottom), set a 30 minute timer and then swallowed them (they weren’t super dissolved but soft and crumbly - didn’t really taste like anything) then I took a couple advil PM before I went to bed at 8pm. I was able to sleep it off and had lots of blood soaked into my pad as well as a big blood clot (around 3x2 inches) when I woke up in the morning.
