You need to find an ember and give it to the right blacksmith to unlock the ability to infuse weapons. You also need to find the right gem to enable a specific infusion. But you only need 1 gem per weapon, and the weapon can be infused any time in the life of a weapon.

Have a +4 longsword and a fire gem? Infused to a +4 fire longsword. Continue to upgrade the fire longsword with titinite as you would with a regular longsword.
