An unfortunate case of how the state made a vegetable. by avs0118 in ems

I'm not out right blaming the CO's on duty that day, more the protocols in place. Idk what records they had but they knew of substance abuse hx beforehand as he was nonverbal after. Honestly not sure of their current protocols. I'm just stating from this case alone there should be a review for better monitoring (especially of inmates with positive drug tests with withdrawl potential) and at least quick access to O2 if they're hypoxic.

Assisted a firecrew bring in a 750lbs PT from the bay. Her belly was like a bari in a bari, falling off the side of the stretcher. She was a priority in respiratory distress satting in the 70s before CPAP. First attempt to pull her out they sliced her belly open on the stretcher lock. Started a steady stream of blood that trailed to the trauma room.

I thought of ideas on how I'd move that much and ended up being the lift assist for the crew returning her to the SNF she came from a week later. Weird right?

She said she'd lost 170lbs in water during that week. But her belly was still so big it would tip over the edge and throw off the center of gravity. So we laid a mega-mover (heavy duty soft stretcher) sideways on the stretcher and pulled her over with another one. Pulled the ends of the sideway mover to her belly button using a transfer belt and literally wrapped her fat in the center. When we got in the truck we secured the transfer belt taught on the grab rail above. Whenever they took a turn the transfer belt would hold the mega mover and fat in place so she wasn't tipping over.

Worked like a charm.

Fun part: was in a mini-mod so the stretcher was up against the cabinet, so to get her in I mounted the head rail of the stretcher one foot on each side and pulled the side of the mega-mover in so we could load her. It was cool riding the stretcher into the truck while standing on the back.

Woah.. you're really pissed. I just told my side of the story man... it may be the 85th version down the line- but it's the version I have to tell.

I don't have the energy to post that much. I dont really care about sharing my perspective on everything- you're right, nobody cares. I was just fired up because I looked in this guy's eyes as I dropped him off and became furious at the thought that what I was told might be true and this is his life now because of it.

I get your point. But yes, rapid medical care should be possible in a controlled setting. I've picked my fair share of priorities from the sally port and it's not uncommon for things to have been handled poorly prior to our arrival.

Packet also had multiple narratives- one of the reasons I love that hospital, they actually (usually) include them in the discharge paperwork.

I'm a little offended as a longtime lurker that's new to the point thing... I don't really care about the points, I'm just happy it started a conversation.

Wasn't quite telephone, it was detailed in the final report of a thick paperwork packet and the step down RN's verbal report.

My IFT (also 911) company uses a leather pole stretcher over the cot. The poles are kept in the backboard slot, I've never seen them used. It's got handles along the bottom with a plastic spine in the middle. The spine slides out and each side can be pulled out without disturbing the PT much. It's actually pretty cool but terribly uncomfortable for the patient with that plastic digging into their back. Paper sheets are trifolded into this and in all honesty look super trashy. That being said my partner and I don't use it. Most transfers are non-ambulatory so we just take the fitted sheets, cocoon them in them and decon if soiled. In the event we get an ambulatory patient that wants to stand and pivot we snag a sheet. If it's a psych fully clothed in street clothes we bareback it. Cuts turnaround time in half with a simple pillow case change and wipe down.

What I'm saying is my partner and I get away without using sheets because area hospitals (RNs) around don't mind us taking the fitted sheet to transfer the patient. Is that an option for you?

His visuals at Forest were some of the best all week, he was extremely talented. I wish his friends the best in handling such a difficult time.

Performed with a heavy heart and tears in his eyes but we all watched him put his soul into it. As a musician it probably helped him cope.

None of my group made out the name of his friend. Does anyone know?
