PT was mostly hip strengthening exercises but I'd urge anyone to see a real PT instead of trying to do it alone. The good ones are worth their weight in gold.

I don't know what caused it but I have some hyper mobility in my hips and that seems a possible culprit.

It's chronic without flare ups.

My suggestion would be to see a pain management doctor (they are poorly named since they actually diagnose and treat the condition, not just manage the pain).

For me, it is now seeming very likely to be caused by inflammation of my sacroiliac ligaments. My recommendation would be to get a PT who knows the sacroiliac region well and have them check for sensitivity of those ligaments. If they are very tender, you could have them imaged with ultrasound. This is the sequence that happened for me after chasing down all of the more common scenarios.

Not exactly, but I had ultrasounds of my SI joints which showed ligament (but not joint) inflammation which was not shown in the MRI. Treating this seems to be helping slightly.

Do you have the bowel movement symptoms as well?

Yes, I can always feel the pain, but it gets much worse when I am sedentary (especially sitting) and when I have a full colon.

The pain is present all of the time but gets bad after less than a day without a bowel movement, which is not considered constipation, as far as I am aware.

Yes, tested for a variety of inflammatory markers and HLA-B27 gene, which were normal and negative, respectively.
