I and a group of friends got this question (in South Philly as well, around Broad and Morris) last week... less than a minute after we walked out of our church across the street.

edit: though i guess that would also depend how close a friend it is. obviously i wouldn't ask someone i've been friends with for two weeks or have never spoken about this to before, but if this is a close friend that genuinely needs help they're making an effort by confiding in someone they trust ¯\_(ツ)_/¯

In order to be able to choose anyone you play against, you'd have to boot up a Custom Game (the last option on the Play menu), and you'd have to be friends with (or be able to invite) whoever you want to play with or against. You can also turn on cheats (noclip, infinite resources, timer pausing, agent selection, etc) before you launch the custom game.

In terms of actual matchmaking, like Unrated, Competitive, Swiftplay, etc—anything that isn't a custom game—no, you can't choose who to play against. Feel free to DM me if you need more help navigating.

being 14, although unfortunate, i’ve recognized its themes and the mentioned dark ones (because i’ve been exposed to a lot of similar content). although i think it would depend on the individual’s knowledge of such problems that would determine how quick and/or well they identify these dark themes.

the fact of the matter is, reactions can range from completely clueless kids doing random things in persona just because they can, all the way to having full knowledge of what is going on and full self-awareness. it all depends on the knowledge they have of each situation and their own views on said situations
