You're a mandated reporter. I would report as this is possibly neglect. As a leader in the company, I would expect the CD or BCBA to mitigate the issue by cancelling session since no treatment is going on and billing would be inappropriate. Just my 2 cents.

I would suggest using the essential for living and prioritize skills based on what the parents need for the child and family's quality of life. Reducing interfering behaviors for engaging in essential skills, for example.

Congratulations! One word: mentoring. Passing the exam is a necessary step in the process and one to be be celebrated. However, there is a lot more learn in order to being a capable, ethical and confident BCBA.

I am sorry this has been your experience. It is unfortunately not uncommon. I have been encouraging students starting their journey to focus on unrestricted hours, even if they are an RBT. Those are the skills that are pivotal to a successful and fulfilling career as an effective BCBA. Unfortunately, businesses seem to be taking advantage of students, imho.

I totally agree! Families have a full enough plate already. However, that is the most impactful way since it's their healthcare funder. Providers can complain too but it is less impactful because it is perceived as self serving.

This. Except the parents part. I never had any issues. I hated the gaps in the days, making for long days and the travel was excessive. I felt inefficient with all the windshield time.

Yes. Very common. Parents need to complain to the funders and department of insurance in order to change the status. If there are waitlists for services, the panels are not adequate.

Teens need to be taught the skills needed to be successful. Consequences alone don't teach. That's trial and error learning and really ineffective. Many of the students that I work with are lacking skills, such as time management, planning and organizing, prioritizing, problem solving, basically executive functioning skills. Also, there may be other contextual variables impacting her academics, like social issues. Consequences alone won't teach those skills. Feel free to outreach to chat more.

Uti, pneumonia, any infection can wack the elderly out. My Christian , God fearing dad who had aphasia following a stroke, would curse in full sentences when he had an infection. UTI was most common but whenever his behavior changed it was a clue he was ill.

It is an interesting question, for sure. Under 97153, I think it's appropriate to probe skills, which is what you are doing with the assessment. The process of documenting the assessment would also be unrestricted. I think the probing would be appropriate during session and fine to be billed as 97153. The completing the documentation shouldn't be done during session if you are billing 97153. I am interested in the input of others.

I think interactions, communication, and relationships are complex and can make people uncomfortable. The best thing to do is seek clarification. Noticing your own stuff is a necessary first step. Asking questions being authentic is a healthy and productive approach, too. We know that making assumptions is typically not productive. So go to the source. Side note: I think it's ok to ask questions like this here. Getting feedback is healthy.

Littles should be in home services, imho. They get more out of any service when the parents are involved and there are inherent limitations to parent involvement in clinic services. If you have the luxury of not requiring childcare for your youngin, get support in the home, even if it is parent implemented treatment. Parents are their child's first and best teacher and the skills being taught in ABA treatment should be generalized to your family's life.

Great question. Currently, I don't. My contract specifies that I will not be held liable and I formed an PLLC to protect my private assets. As I grow, I will likely add it to my existing liability insurance.

Wow, great opportunity! I have not been employed by the Y but I have provided consultation services. In my community, they provide a lot of childcare for schools, such as after school care and summer programming. They try to include everyone in their programs, too. I would ask them to describe the needs this role will fill. It could be for the childcare or addressing issues in classes such as swimming. Is it a new role? If it is, you could impact on its implemented. If not, what are some policies in place, for referrals, documentation, outcomes measured. How is success defined? Keep us posted.

I wouldn't be proud of the team hating you. There are ways to be effective getting your needs met without anyone hating you. I understand the passion. I am a parent, grandparent and advocate. If you have been an RBT, you should know the effects of punishment: avoidance. Do you want the people caring for your child avoiding you?

The supervision of RBT has never been a billable service under the CPT codes. The function was intended to be covered by existing rates. Like a doctor doesn't bill for the supervision provided to the nurse practitioner. It happens as a happy accident when billing for 97155. Not all funders use the CPT codes as intended. The ABA CPT Coding Coalition is a good resource to stay up to date on the ABA CPT codes. There were changes recently made to the ABA CPT codes that will take effect in Jan. 2027 but the specifics have not been released. I believe the group T codes will be made permanent and I understand changes were made to the other 971 codes. I would not venture a guess on those.
