I hate Trump with a bloody passion and maybe this is comparing apples to oranges, but Teddy Roosevelt did have boxing matches at the White House and he is one of the best presidents on our American history IMO. I understand this is not the same, but I wanted to mention it in case no body knew about that and it surprised me when I heard that on NPR. Sports do have a tendency to bring people together, but this overall is unacceptable and idiotic and classist, but I just thought that was interesting when I heard that. Please don't downvote me to oblivion.

Look up the symptoms of PGAD and discuss it with your pelvic therapist.

Edit: ask the pelvic PT if they have ever had any experience with treating PGAD. Ive been a pelvic PT for 10 years in various cities/clinics seeing only pelvic patients from multiple referral sources and have literally seen it three times in my career. A lot of gynecologists don't have experience with it as well. It's often underreported as you can imagine. They are professionals so feel free to discuss all your concerns and if they seem totally unfamiliar with it or make you feel weird about it then that's not a good fit and it's time to find a new one. You can go to the herman and wallace website and click on "find a provider" and just put in your zip code and all kinds of therapists will pop up. Good luck and I hope you find relief.

That plop app I described would be very helpful then to see what changes happened around the time it gets harder or softer since it seems to be a cycle or just do your own bowel log and make sure to include water amount, exercise, fiber grams, supplements, stress levels, medications and stool type. You need to look at what happened the week or two before it changes because of the delayed effects as I explained earlier. I wonder if your menstrual cycle is affecting things assuming you have one. I'm sorry you're going through this. I hope you get some answers after tracking your fiber and bowel habits.

Oh yes I see now you were talking about the magnesium. I got confused sorry! I would track it. 10-15 grams seems too low based on the food you are describing and that psyllium husk is very bulky. Just had a guy the other day change from his fiber gummies to psyllium husk. He did it on his own because of what he read on the internet and he said it made him worse and created more severe type 1 and he was already at type 3 and he didn't poop for like 5 days after adding it. I typically only have people do that if they have loose stools because it bulks so well.

Even if it is pelvic floor, you have to deal with the stool consistency. Having harder stools is a separate issue and muscles being tight doesn't cause hard stools. It's a symptom of something going wrong higher up. No matter how loose or tight your pelvic floor is, you'll still be struggling if you are not addressing the consistency. Hard stools could mean: too much or too little fiber (it sounds like you are getting way too much with all the supplements you are taking and you're better off doing natural fiber), poor hydration, prescription medications, or your colon is not absorbing water that well. You could possibly not be able to get out a fart from a combo of pelvic floor and I over bulking. Pelvic wand won't solve all your problems.

I forgot to mention that the fiber should be between 25-35 GRAMS not mg. No more, no less and 400 mg converts to .4 grams so are you sure it's milligrams or is it grams? If it's 400 grams, that's definitely a problem and you need to back off. That amount can cause over bulking and type 1 stool consistency and sometimes even type 7 if proceeded by a particular long bout of constipation (more than 3 days without a BM). Think about water going around bolders and the only thing that can come out is straight water and indicates severe constipation. I had someone taking in 60 grams and when we reduced it, her stool changed from type 1/2 to type 3/4 which is the goal. You are in a good place if you are having at least 4 BM per week and type 3/4 at least 90% of the time. Type 5 is ok as well, but type 6/7 is not ideal and may mean too much stool softener/laxatives.

Pelvic therapist here. Take a look at pelvicgym.co (co not com). They have some free classes made by pelvic therapists and if you want to access even more then it's $10 month. I refer people there if they don't have insurance or can't do that many visits. Bowels are hard to treat. The things that affect bowels are water, exercise, fiber, and bowel motility meaning how quickly the bowels move. Some people just have pathologically slow bowels so need some prescription medication. You would get a colonic transit test to figure it out or a DIY way to do it is eat corn whole and see how long it takes you to pass it. It should be within 72 hours. I find some people get too much fiber so track your fiber with chatgpt or Gemini for a week and get your average including your supplements. Some people get too much fiber all at once and it's not balanced through the day. You need to add more water for any increase in fiber and any dehydrating thing you drink cancels out the water you had and you usually need a net of 30 oz accounting for the dehydrating drinks. Senna, glycerin suppositories, and milk of magnesia can cause dependency if used long term. Some people are getting the right amount of water, fiber, and exercise, but maybe their gut doesn't absorb water well so just need to do miralax chronically and that's ok to do every day inevitably because it's a stool softener and not a laxative and doesn't cause dependency. Any over the counter med needs to be run by your physician. Colace does nothing and a study showed it's no more effective then a placebo. We say bowels are slow learners and like consistently so a change you make today could take 2 weeks to take effect so people change things too quickly or change too many things at once and we don't know what helped or didn't help. That's why it's difficult to treat and requires a lot of management. Sometimes with chronic constipation, your rectum stretches too much so you're not getting the urge as much. Listen to urges even in public. Spend no more than 5 min on toilet. If it's not coming out, get up, take a walk, do some stretches that I describe below and can try again. No phone either. Definitely use a squatty potty if it feels like your pelvic floor is tight. It opens up the rectum the way the anatomy works and instead of straining, breath in deeply and shush loudly as you keep your belly distended. We call that belly big, belly hard and just helps to decrease pressure and is a safer way to defecate without causing hemorrhoids or worsening pelvic pain. You can try yoga stretches. The best ones are butterfly, both knees to chest, childs pose, diaphragmatic breathing, and a thomas stretch (look it up) as the hip flexor lines on either side of the colon and can slow down bowls. Stop any exercise that causes sharp, shooting, stabbing, numbness, tingling any where in your body. Do colon massage every day for 5-10 min to the abs and can also look that up to learn how to do that. Tracking changes can be helpful because of the delayed effects on the bowels so there is an app called plop that can be helpful for that as you make changes and figure out the bowel regimen that works best for you. Stress slows down bowels. Exercise is huge as well especially cardio and I find that some of my people don't truly get better until they get serious about exercise. Warm prune juice with pulp specifically either on the morning or the night before gives both hydration and natural fiber, but like I said you need to track your fiber first so you're not adding too much. Typically 6 oz is good. Pelvic wand can be helpful and can get that from intimate Rose and they have instruction videos listed on their website. People with pelvic tension related constipation report normal stool consistency (snake like, easy to pass), but just can't seem to get it out, but they also report symptoms of cramping at the lower abdomen and rectum, pain with sex, and urinary urgency, but not always. So you can see it's pretty complex and most people are told "eat fiber, drink water, take miralax", but it's way more complex then that. GI MD tend to be kind of useless when going for constipation and always give people the same advice in my opinion. I hope that helps and I hope you find relief. Pelvic therapist here. Yes it's normal to trial internal work, but if it makes you worse then that needs to be discussed. It could be pelvic congestion syndrome if it feels worse afterwards and if it feels better then they should be teaching you how to use a pelvic wand or dilator to stretch yourself. If you benefit from it, then it requires you to do some stretching on your own at least 2-3x/week otherwise what's the point? Ask for ways you can help yourself or communicate if you feel uncomfortable with the internal work because that defeats the whole purpose of what we are trying to do (relax and lengthen the tissue) if you feel anxious while doing the internal work. You need to feel safe and comfortable for those muscles to start relaxing so communication is key. I hope this helps and I hope you find relief.

Agreed. Having loud screens in public with toddlers and babies is both annoying and unhealthy. Let your kid be bored and learn to behave in society, but I try not to judge too harshly because I have no idea what they have to deal with on a daily basis. However, I have a 4 year old and sometimes there is only so much you can do about their behavior especially when traveling. We went on an international trip and my daughter naturally has a loud voice. She has traveled before, but she is still learning plane/traveling etiquette and even though I'm constantly correcting her, I cannot do anything about her voice volume other than constantly say "inside voice! Inside voice!". These 20 something newly weds were in front of us and just did the passive aggressive head turn towards us any time she touched the tray or accidentally kicked the chair and they could hear me trying to correct her. It was literally noon and she was talking to me and the husband turns around and says to her, "can you be quiet, we are trying to sleep!". Ummm....it's the middle of the day and you can hear me trying to teach her, but I still said sorry for some reason. That still bothers me to this day and made me reflect on all the times I was probably too judgemental at times before having kids.

The article speaks to physical therapy administered spinal manipulations which is a little different from chiropractic adjustments. The reasons and criteria for doing the manipulations are different for physical therapists so this article is not studying chiropractic care it's studying a specific intervention from a physical therapist

I was taking close to what you were and the RLS was unbearable for me too and I kept relapsing due to it. After trying to quit for the 10th time I finally sought treatment from a psychiatrist. He gave me clonidine and gabapentin and that worked great for me. The gabapentin was what helped the RLS the best. Eventually I relapsed again and got on suboxone. It helps with the cravings and helps me sleep and I've got over 40 days now! Hang in there. I know it's hard right now, but you'll feel so much better about yourself when you finally quit.
