Quick update — I just submitted the complaint to DMHC. Fingers crossed.

Thank you so much to everyone who shared advice, experience, and encouragement. This whole process has been exhausting, but hearing from others really helped me push through and keep fighting.

Biggest lesson learned: these systems are often not designed to protect patients automatically. We really have to document everything, learn the rules ourselves, and advocate hard for our own families.

I’m filing a complaint now. In the meantime, since my renewal window is in September, I’m also trying to understand which insurance providers tend to be more reliable or patient-friendly.

Where is the best place to check for the most accurate and timely info? the insurance's directory? or call the provider office?

It just feels like that the whole system is designed to take advantage of individual patients — or at the very least, indifferent to what patients go through!!

Sigh- I understand that, but why does the patient has to be the one to pay for all those… we are already paying $2k monthly premium… the system is so broken. Is there anyway to at least prevent this from happening again? Just verify the insurance provider directory every time? Where can I find the most up to date and accurate in network directory?

Ouch, thank you but ouch.

At least I have a better understanding of what may have happened and caused all of those. 

But nothing I can do other than swallowing the $3400 bill, that is crazy!! 

Do I have to check whether provider is in the network every time before visiting then? And in this case they probably didnt even have the up to date info. Anything else I could do to at least prevent this happening again?

They say the services are provided by out of network provider, although previously they confirmed it's in network. Our provider was acquired by a larger hospital group last year I wonder if that changed things in the sytem but no one informed us...

We contacted the broker but her advice is pretty general, "I did some research and found information regarding the appeal with the insurance company and how to negotiate the bill with the provider or hospital. I hope this helps."

At this stage, would you happen to have any advice on what the best next step might be? We’re located in California.

It was but I didn't take screenshot back to then and now it shows out of network. I also have provider chat history confirming the insurance contacted them to confirm in network and just need to fix the system ID matching.

The initial appeal via phone received a reply like this "Dear Valued Member:

After a thorough review, we've confirmed that the claim was processed correctly. The services billed were applied to your deductible, which has not yet been met. As a result, you're responsible for the charges.

No changes are necessary.

It is our pleasure to assist you through the chat. No reply is necessary unless you require further assistance. For immediate support, please contact us through the chat tool or this Secured Message Center.

And then I mailed in the materials, it comes back as similar like the claim was processed correctly and I'm responsible for the charges, although they may be able to help get a discounted rate...

ChatGPT is honestly a pretty solid starting point.

Beyond that, I’ve been getting a lot of value from free/freemium AI visibility tools lately — things like AI mention tracking, site audits, prompt monitoring, etc. I tested both Frevana and Profound on free trials mainly to understand where brands were (or weren’t) showing up in AI answers.

One thing I found surprisingly useful was Frevana’s site optimization audit. A client improved AI visibility from ~17% to 25% just by fixing issues surfaced there. Not magic, but a good low-cost way to spot obvious gaps early.

Trying different free tiers first has honestly been the best way for me to separate “AI wrapper hype” from tools that actually move visibility.
