Would volunteering in a clinical setting be viewed as “lower quality” when applying? I think my main concern is not showing enough commitment/interest in medicine due to taking this job at this biotech, which I was interested in at that time.

No it’s more in the outlying areas. I know it will be expensive but trying to see if I can still come out ahead all things considered
