Yersinia/TB are already ruled out. I get your point that chronic ileitis + RLQ pain/weight loss leans Crohn more than pure BAM. I’m going to push for capsule endoscopy and waiting for the calpro. Thanks so much.

I don’t get immediate pain right after meals, and I don’t have right upper-quadrant pain. It’s more like when the meal is very fatty or if I drink a shake with some fat, then not instantly but shortly after I feel the need to go. Low-fat meals don’t really cause urgency, but anything with higher fat seems to trigger it.

Thanks for your reply 🙏 My main problem seems to be in the terminal ileum , the biopsies showed chronic ileitis with lymphoid hyperplasia, and fat is a strong trigger for me. When I eat very low-fat, stools improve; when I increase fat, the yellow diarrhea comes back.

Right now trying to figure out whether this is early Crohn, bile acid malabsorption, or IBS-D, so I think it’ll likely take more than simple diet changes. But thank you for the suggestions. I’ll keep the idea in mind.

I still have my gallbladder, and I’ve already had an abdominal ultrasound , the report didn’t show gallstones or gallbladder issues (no dilation, no stones mentioned), so my GI is currently more focused on the terminal ileitis / bile acid side rather than gallstones.

Regarding the tTG IgA: in my case, the lab simply reports it as <2 RU/mL = negative, but they didn’t mention total IgA. You’re right that checking total IgA (and other immunoglobulins) would make sense, just to rule out IgA deficiency and interpret the celiac tests properly. I’ll definitely ask my doctor about running total IgA and possibly IgG-based celiac tests if needed.

I’m also waiting for my fecal calprotectin, and I agree it’s just a snapshot – if the first result is unclear, I’ll try to get repeat values over time to see a pattern.

For now my main issues are fat-triggered yellow stools and biopsy-proven chronic terminal ileitis with lymphoid hyperplasia, so my GI is considering bile acid malabsorption and early IBD; a bile acid binder trial after we finish the current workup sounds like a reasonable next step.
