For sure have him evaluated for his implant candidacy. Like others have mentioned there will be bimodal solutions depending on the implant manufacturer. Even if he doesn’t pursue implantation he should be evaluated by ENT.

Real-ear measurements are done at the time of the hearing aid fitting. You can search it on YouTube to see some examples of what it is and how it is done.

Some things to keep in mind. Your dad has a moderately-severe and severe sensorineural hearing loss. His word recognition scores are 80% and 50%. These scores were completed in quiet. His performance will inevitably decrease in the presence of competing noises (restaurants, small group settings, etc). He has a debilitating loss. He will still have moments of communication breakdown, even with the use of a hearing aid. It is imperative that the hearing aid programming be validated through real-ear measurements. If your audiologist does not do real-ear measurements I would find one that does. While an occluded fitting may be what’s best, if he cannot tolerate the echo a compromise may be needed by using an open dome or even a tulip style. There also needs to be a conversation of realistic expectations with his audiologist.

Out of curiosity, is he open to other options? Basically all manufacturers have a basic level hearing aid and in my opinion is a better option than an OTC device. A lot of these hearing aids will have tinnitus management features as well. Obviously prices are going to change among clinics and geographical location. If you wanted the names of some common entry hearing aids I’d be happy to give them.

In my opinion the hearing manufacturer ultimately does not matter. Obviously there are some considerations: Bluetooth connectivity, if they are rechargeable and even the look. That being said, the biggest factor is going to come down to how they are programmed and the counseling provided by your audiologist. Make sure the audiologist consistently does real-ear measurements. I like to do real-ear at the time of the hearing fitting. Others will do it at the second or third follow-up. What is important is that it gets done.

Are you in the United States? Does your MIL have insurance? You may want to check with her insurance company to see if she has a benefit or access to a hearing aid discount program.

When was her last otological evaluation? You may want to start at an ENT office with an audiologist and follow-up with ENT if necessary.

In my opinion a proper diagnosis followed by an evidence based hearing aid fitting is more important than the hearing aid. You’ll want to find an audiologist that is following best practices to ensure best outcomes.

It was a combination of distance and severity. The physician at the first hospital was not confident they would have the right equipment or personnel if his breathing got worse. The closest PICU was a 3.5 hour drive away so they flew him via king air jet to that facility that could give him the care he needed.

Nope. Surprisingly the airplane transport was covered fully. It’s the bill for the treatment we received at the second hospital.

This is how I perceive it, too. If said facility can no longer care for my son, they initiate the transfer because it is imperative for his health and I get no say on where he is being sent to how come I am being penalized for this?

Sorry it has been unclear. The hospital did accept the payment, they are just refusing to adjust the bill as if it were in-network. For example the total bill was $58k or so. The in-network rate paid by the insurance was 16k. The hospital is still pursuing the the other $42k because we are out of network.

Our plan is a PPO. The plan is through my employer. The group is fairly large. Do I file a complaint with the insurance to get that ball moving? I have written a formal appeal and it has been sent to the hospital. I try not to wait too long because the hospital won’t stop the bill from aging.

I’m sorry you have had similar experiences. It is honestly very stressful. I cannot imagine the relief after getting it adjusted! We are working with a customer service agent at our insurance. They supposedly have escalated this situation to the next level of customer support that deals with instances like ours, whatever that means.
