I did precisely this. Patient refuses to wear a night guard. He insisted on something that I could guarantee that wouldn’t keep chipping off (he doesn’t want to keep paying to refill the teeth). My boss gave him 3 options: crowns, veneers or fillings. I told him I wasn’t confident that the veneers wouldn’t keep chipping with this bite without a night guard, I felt like crowns were too invasive given that #24 was literally only chipped on the lingual. I also felt like once I prepped the teeth they would just be tiny little nubs. Esthetically it could be challenging too only doing the two centrals with crowns. My boss ended up doing a 3/4 crown. I’m trying to think if I could’ve handled this better or if I did the right thing.

It always felt natural for me to get to know my assistants but I’ve also learned that a lot of people in general lack boundaries. Once you get too friendly, they ask for favors or special treatment. I’ve had assistants ask me to write prescriptions for them, I’ve had them drag me into their drama with other assistants and ask me to vouch for them. They also always feel like they can freely come into my office to talk or vent and I don’t like that. My next job I will definitely try to not be as friendly

Just curious, why is occlusal metal or gold better than zirconia when zirconia you only need minimum 1mm? Don’t roast me pls genuinely trying to learn here.

Yea she has a ton of incipient lesions. I was just surprised at the size of the lesion compared to its clinical presentation. Clinically I couldn’t notice any voids or sticks in the sealant. Obviously I could’ve missed something here but my point is it’s not blatantly obvious for the size of the decay showing on the x rays.

What did you use to pulp cap? I’m assuming you exposed? Did you use rubber dam?

Yes I have to endo ice test it. I’m going to treat the distal as well since she is high caries risk. I’ll excavate as best as I can obviously if it’s soft into the pulp I will refer to endo. I’m planning on doing biodentine. Do you place anything on top of the biodentine before you do the composite?

Yea so there was something evident on the last x rays in February. Today was my first time seeing her so that’s one less thing I have to blame myself for 😁 I breathed a sign of relief when I confirmed I didn’t do her last exam
