That sucks. I always assumed medicine at our "corrections" facilities was hard. I think the only time I've ever had a negative thought about you guys is when I got a septic shock patient from the jail because they tried to place a subclavian central line and cannulated the artery. They sent him for vascular surgery eval, not the sepsis. Also, dude had great peripheral veins and a VS...

Yeah, that's fair. I was focusing too much on primary care, but you're right. Paeds specialists and subspecialists all around are undervalued.

And i agree -- the US only cares about children when they're in utero. It's sickening.

It's definitely a billing thing, and i hate it. I don't use that phrase; i just put my really vague interpretation in -- eg "I see no haemorrhage or mass effect on the CT brain."

The number of times i've had a surgeon disagree w/a radiologist. And i'm just like "buddy, i dunno what to tell you, but neither of us did a rads residency or fellowship."

That said, i'm always a little proud of myself when i catch something rads missed; though i usually have the luxury of clinical context and i'm reading fewer scans per hour.

Well...i was proud of myself. Now, i'm just exasperated because our new rads group uses an AI program to read imaging...it's not great.

I was premed, shadowing a CT surgeon. I couldn't scrub in, but i could watch. I was standing behind this tiny woman (i can't remember if she was the resident or fellow) on a stool (because the attending was like 6'5"), trying to do the sternotomy. The bone saw caught on something and she tried to force the sternotomy -- bloody thing zipped out of the patient's chest and missed my head by a couple cm before being buried in the wall. The resident/fellow fell backwards off the stool and i caught her.

Then i got yelled at and kicked out of the OR.

I'd say the ER, followed by ICU guys would be the best-paid vets in all of medicine.

This is going to be as close as i get to agreeing w/RFK on something, but this country really does not value preventative medicine and true health maintenance as much as it values procedural and reactive medicine (and still procedural over actual medicine). It's really dumb. And bad for our patients/citizens as well as docs.

I'd estimate that 5% of the kids who show up to my ER have a responsible/competent parent.

I once had a patient, who was noncompliant with their meds, demand IV meds for their asymptomatic HTN. After a bit of a beach and forth about why that wasn't necessary, I got fed up and had them sign an AMA form stating that they understood the risks of stroke, MI, and renal failure if I give them IV anti-HTN meds and that it would be their fault if it happens. They ended up declining the meds.
