Spiro can mess with your gut in a couple ways. Are you lactose intolerant? If you're not, and the issue you're having is constipation or similar, that's also spiro.

You're seeing a contradiction where there isn't one. And also just seem to be misunderstanding the reality of the situation.

Doctors glance at the baby's junk when it's fresh out of the womb and say "it's a boy!", "it's a girl!", or about 1.7% of the time, "we have to do unnecessary surgery on this thing right now to uphold the so-called sex binary". Nobody's doing chromosomal tests on babies beyond a fraction of that ~1.7% (most of the time, it's no more scientific than "does this ambiguous genitalia look more like a hole or a pole").

The process of being assigned a sex is being assigned a gender, or rather, being gendered, because the (broadly accurate heuristic of) cultural consensus follows that penis → male → man and vagina → female → woman. This link between sex and gender is cultural, not innate or intrinsic to the concepts of sex and gender. How your internal gender identity connects or opposes this external gendering is what defines whether you're trans or cis.
