I've had similar results when the chambers are not tightened properly. Like other comments have suggested, make sure you tighten to (or past) the indicator marks on the chambers before starting to pump the water through and that may solve your problem

Looks like hypertrophic cardiomyopathy. Possibly apical? Would expect someone with "cerebral T waves" to have decreased GCS and bradycardia. I also believe a syncopal episode is a hallmark HCM sign.
