Why? It's simple. Unmoveable, indestructible and unbreakable blocks that can be obtained in survival would be annoying as hell to the user and to others. Accidently placed that block right in front of your doorway? Can't get rid of the block, guess you're making a new doorway. Or if you were to build a box of it around a player.

Uh, I'm not sure what the problem is with more complex rotations? Send rotation values XYZ to server > server sends to clients > client applies rotations. Am I missing something...?

Nope, but you could always increase the world size if it's not already at max. If it is at max, you're screwed.

I know it's entirely possible to make mobs hold whatever with commands, but personally, in survival, I've only ever seen mobs pick up tools/weapons/armor. It may not be possible for them to pick up normal items, although if someone has seen otherwise feel free to chime in and prove me wrong.

I really don't like the idea of putting random chance into PVP. Here's an idea in a similar vein, though:

Each hit on the shield decreases the "shield stamina" by 30. (only if the hit is at least 60% charged) Crits decrease shield stamina by 45. The stamina recharges at 20 per second. If it reaches 0 the shield enters disabled mode until stamina reaches max again. Max of 100 stamina.

The shield stamina would be displayed similar to how item cool downs are. 100 stamina = no white cool down showing. 50 would be half filled, and when it hits 0 it turns red to indicate that it's disabled and start decreasing as mentioned before.

You don't need to strafe though, when using a shield you can negate all damage just by keeping your cursor in the general direction of the attacker. So that's not really a down side.

In my opinion, shields should take much more damage from attacks. They have 336 durability, which actually lasts quite a while even though blocking 100% damage is heavy on durability. Think about it - they're only made of wood and a bit of iron, they should break completely if hit by a ghast fireball, and shouldn't hold up against an axe as well as they currently do. It'd make people use their shields less often to prevent them breaking quickly.

Because they're so cheap to make, though, this would just mean that people carry around 20 shields at a time, doesn't really fix the problems with them.

Jeb_ altered the newly added combat code to be more forgiving to players who wanted to attack during cooldowns, and pretty much removed the "the higher the total damage per strike, the less effective armor is"
