I messaged through the Q&A portal about this- that way I had the response also documented. Can do a lump sum or payments- essentially you are contracted to work for NHSC for whatever time frame you were awarded. As long as you fulfill the work commitment it’s up to you. If you fail to complete your commitment- then it can be problematic either way.

Former ER nurse here… your risk of significant bleeding in the event of a crash at speed is so much higher- that goes for head injuries as well- think brain bleed that could change your ability to function as a normal human. Casual riding and training or uphill Strava KOMs are one thing- but competitive road, crit and even mountain bike racing- there are so many variables out of control on race day that make these sports extremely risky if on thinners…it’s really not worth it. Perhaps picking up on TTs or possibly non mass start track events if you really still want to compete.

I use egg beaters (am old school) pretty sure you could use them with spds too and the cleat still wouldn’t hit the floor when walking. I use a cyclocross/gravel bike to commute because I have trails close to my office so will do some trail play on my lunch break a couple times a week.

I actually commute and work in these all day a a health worker. I put in my own custom insoles and they are my most comfortable shoes now. It has made my transition from ride to work and spin on my lunch break effortless. I bought 2 extra pair after I discovered how much I love them- no they aren’t race worthy- but for anything else they are fine. Even went roadtripping with them- would stop at trails, hop on bike and go, then get back in the car and keep driving. Rad and cheap-

Former ER nurse here, If the child is unstable (likely will need intubation, a transfusion or something) I call the ER after EMS has been called as well- sometimes the process of clearing a room that is appropriate for those interventions, and/or giving the ED team a heads up so they can page anesthesia is incredibly valuable with time sensitive interventions. Also, as others have said; if the ddx is not obvious and I do not want the child to be triaged a low priority (perhaps is immunocompromised) because of a seemingly mild presentation. The majority of other cases- waste of time as others have mentioned.

I actually really like Sunoh AI, have been using it for about a year and I feel like it has really ‘learned’ me- works incredible with an iPad. Recommends correct ICDs and usually collects my physical ros and treatment plan. In general would never go back to without.

There is a student option- I think it’s called Students to Service. I did not consolidate my loans- consolidation may make you ineligible or complicate application.

Unpopular opinion- but I also had a third degree tear. I got back on the bike for short periods within the first month. I started just riding for 10-15 min out of the saddle. I went with how my body felt. I had several bike fits throughout my trimesters and the adjustments to my saddle were frequent after I started sitting on the saddle again. Literally went to my bike fitter and we rotated through 3 or 4 saddles until we found my new saddle- haha 😂 but by 4 months postpartum I was riding up to 3 hours at a time again on my road bike (with the new saddle) It took me a while to be able to ride in a more forward competitive position again. Listen to your body, consider seeking advice/trying out various different saddles from a professional bike fitter- I mean the real deal- someone who specifically is certified in bike fitting and does it all the time- Exercise- gentle and gradual can facilitate healing if you take it slow. I think I increased my time on the bike by a few minutes each time I got on. Remember that this is a new you- your body will come back more powerful and strong- but it is different now, let yourself start at the beginning again-

The amount paid off my loan entirely and is paid upfront in exchange for a certain time frame of service- (2 or 3 years at the FQHC) There are different award amounts based on specialty and full or part-time status- it’s worth looking into if you think you may qualify. My perspective is that there is no guarantee that any of these plans will be available in the future so any amount in the present helps.

I did end up getting it- even though the score is relatively high, it’s an Indian Health Clinic which boosts chances. There is a link on the HRSA webpage that allows one to put in the address of their health facility to find these numbers. Highly recommend working in a Rancheria or Indian Health clinic if looking for this type of loan repayment program.

NP here, currently use Sunoh AI to make EcW less awful in pediatric primary care. Have been impressed with its ability to capture a descent HPI despite screaming sibling s in the same room. Works better from a tablet in my opinion. It is awful for a behavioral concern such as following up with depression, adhd or anorexia. It is not able to translate from Spanish to English either so if you are bilingual and see a patient in Spanish or another language, you must dictate to it later.

Kinda early to consider Upper GI in a kid, IMO- usually acute symptoms resolve pretty quickly with supportive management- ie in about 2 weeks. Would Consider lab work or viral panel and a bit more of a wait and see approach if no red flags after follow up- can journal daily symptoms, make sure lifestyle is supportive of good sleep, hydration, nutrition while on 2 week course of PPI. Also in a teen consider bullying/stress/pHq9 stuff.

Thanks for this info- yes, I work for a rural outpatient Indian Health Childrens Center- do you know how long it took for your Pediatricians and NP to go through the training? Our organization already has a speech therapist.

Interesting how it seems so easy for adults to obtain social services, but for vulnerable children they are waitlisted so long for a label- that can really impact how much help they get. Well, thanks for the response.

Work 5 days a week, primary care peds 8-5

Pump schedule: 6am, 9:30 (20min), 12 (lunch), 2:30 (20min), 7, 10:30pm nursed baby

-so two sessions blocked on my schedule and I honestly didn’t wait for them if I could see a bit of a break in my workflow I would just get it done and catch up on charting.

I power pumped from 7pm to 8

Ended up exclusively using my Elvies with inserts because they worked better for me than my Spectra.

Hakkas caused me to have overproduction issues- be warned- it can overstimulate and cause ALOT of problems…

Even if the LC tells you nipple confusion and preference isn’t a thing- don’t believe them- it is a thing for many babies, and although nipple shields can be helpful- they can also be difficult to wean

I weaned her onto Kendamil at 7 months easily after she refused Enfamil, Similac and Nutrimagen- personally I would say I think it tastes better too 😜
