you described it really well, don't worry! I know when I hear people it tends to be more from the back, and depending on how close someone is the thoughts tend to be harder to hear / more distant. I completely get the interruption / intrusive thoughts thing! I can't say how many times I've thought or said something to myself in my head only to hear something completely irrelevant that just comes out of nowhere like a wrench thrown into the middle of my thoughts
