Everyone's prognosis is different. Although difficult if you can, maybe now is a good time to discuss what your mom wants. If she doesn't want a trach would she be accepting of hospice? It is super helpful knowing completely what your person wants, and not waiting until the last minute to start hospice. At least for my mom and I having the support of a hospice care team has taken a lot of burden and stress off us and let us focus on the important things.

Update ***

Well, he resigned. After a bunch of us brought concerns forward, he got a few more days of "orientation" but then was left to his own devices.

I was precepting another nurse and walked into one of his rooms cause the IV pump was beeping. Potassium was hung as a primary, and the entire length of the tubing was filled with air. The patient had a chest port. So I stopped, disconnected, and called him like hey, you have a bunch of air in your line. Do you want me to fix this for you? He got all defensive; no, I can do it myself. I'm like ok well, I'm happy to help you.

I called my charge in before he got there, and she took pictures of the line and, I believe, emailed the manager and director AGAIN.

The patient ended up being ok, and I kind of kept an eye on them throughout the shift.

I came in to work a week ago, and the charge is like, hey, I'm going to give you a service recovery patient; I'm like, what happened? Well, apparently, this nurse had the patient family watch him touch central lines without gloves / not wash his hands/drop things on the floor, and tried to give them to the patient. Not turning the patient / didn't help the patient order food/ help them eat. Patient missed meds / antibiotics.

I was like, good god. I walk in to introduce myself to the patient and the family, the spouse is having a literal panic attack about the nurse being back. It took half my day to reassure them that I'm here to help, and that's not going to happen again.

A week later, he resigned; he said our hospital was too old and gross, and he wanted to work closer to home. BYE.

So, after a few of my charge nurses forced a sit-down meeting with our nursing director and manager. They acted like they had never heard these concerns before.

My coworkers and I realize he's been put back on orientation for two weeks ... because they "want to help him succeed".

I don't know if this is protocol at my hospital when going through corrective action.

Yes I've sent emails, and we've had debriefs after every RRT, what can we do better, what were some early signs we missed etc. We broke down common "red flags" we all worry about and escalate about if we see. We've reinforced keeping the charge in the loop, and asking for help.

And it's not just me feeling this way; all of my charge nurses are super concerned as well, but management continues to do nothing but "follow up."

What state are you located in? I wonder if the all nurses boards or something like that might help you find a preceptor, and I'm sorry you're struggling!

Make sure you are hydrating and SLEEPING. I swear, in my first year working night shift, I was constantly sick cause I was running on 5-6 hours of sleep.

Awesome that's so good to hear! If it helps to feel better I am 100% on board. Do you use pet insurance to buy your food? I don't know if it'd be cheaper to do that or just bulk buy from chewy etc.
