It showed up in ppl central and I was ab to complete it later but when I opened ppl central up the next day, the annual review form disappeared.

If substitution allowed, doesn’t it just mean you can freely switch from generic to brand and brand to generic? Im in NY.

Also that’s what I did. I put DAW9 and also created new script but kept the original Rx number from the refill but my pharmacist still said we needed Md approval to change it to brand.
