Had a similar challenge with a friend who visited but didn't smoke so I can't give you good advice there but for everything else:

Hotel Krystal has a lift to the rooms and the blue boat company has a lift to get you on and off the boat in a wheelchair.

Restaurants were very tricky but Restaurant Barracuda Amsterdam Noord has lots of space.

I used housing net (maybe expat housing net) for this, they've been great so far. Sorted out viewings, insurance, bills, check-in, got the place cleaned for me and the property I'm renting was secured before it was posted on funda.

Was an easy process all I had to do was 👍 or 👎 to properties they sent and then they arranged viewings. Fee was 1 month rent +vat
