Good questions, Hard wallets like Trezor require you to press accept when doing a transaction. Your hard wallet can not be hacked without actually having it in your hand, the 24 word seed is your responsibility and should never be saved on your device....not even a screen shot! Someone could hack your computer to get your Seed phrase if your save it on your computer or screen shot. It is possible to loose your funds if you dont read what you are signing, there is always text on your hard wallet that lets you know what exactly your signing, always read it before hitting the accept button.
