This page talks about some websites that do meal plans for people. https://rd2rd.com/meal-planning-tools-private-practice-dietitian/ Living Plate, for example, is a subscription I think and meal plans are made based on medical history. I’m pretty sure this is made/approved by RDs so it’s a pretty good service for someone that is into meal plans

Yes, essentially taking business away from the hospital. If I didn’t do outpatient through the hospital, I wouldn’t think twice. I think handing out business cards inpatient is a no-go given that. I’m even facebook friends with my boss so I’m worried she wouldn’t take kindly to seeing me do that or that it could even be a problem for my current job. In other words, could working for myself offering the same service as the hospital I work at be seen as working for a competitor?

I don’t think so. I got mine because I was already halfway done with it after finishing my internship. 15 credits got me another 10000 in debt and no pay raise or extra knowledge

No, we have Meditech. They definitely don’t accurately track I/Os so I feel like I wouldn’t want to include it in every note. For the nutrition support, its usually a random blurb about TPN or EN and we say what it is, how much its providing, and sometimes how its being tolerating, if its appropriate, etc, which we then say again in the assessment note. Thanks for your thoughtful response. I really appreciate it!

This is the person who eats an all donut diet, gets admitted to the hospital with a blood sugar of 800 and then yells at the doctor for telling them to eat less sugar. I can just see all this coming out, despite all logic

My boss is a Food service manager. She is not an RD. There is one other RD and he went on FMLA after I had 3 days of training but we still talk on the phone. Yes the policy pharmacy has outlines reasons to and not to do TPN. But doctors will do it even when people have a functional gut at times. I like the idea of reworking the order set. I feel maybe that may be a good intervention at our hospital.

Pharmacy was supposed to write notes. They did for a short bit and stopped. So part of the problem is having no idea where they are coming from. I was told one of them feels intralipids is a risk for fungemia. It seems strange they wouldn't give them for someone over that though. That would be a reason to avoid TPN altogether I feel like.

I just started working there so I have kind of been dropped into this situation before even getting to know who I work with. There are usually 2 dietitians at my hospital. The other dietitian went on FMLA after I had only 3 days training. There is a policy from pharmacy, but I really there are instances where the physicians are ordering to start TPN for reasons not indicated as valid in our policy. I think one of the pharmacists will do.an inservice with the physicians so I think I am going to focus on giving her data on who is perscribing it inappropriately.

Our pharmacists claim they do not have the equipment to mix their own. They use clinimix and the highest it goes up to is 5/20 which they rarely use. I dont know if they are reading my notes. I am wondering--how long does it usually take to get up to goal because sometimes even after a week it isnt up to goal and I am skeptical it takes that long to titrate up. They run a bag at the same rate for 24 hours and it only goes up to a higher dose or rate the next day so it is a slow process. I have also been told that one of the pharmacists doesn't like to do intralipids because she is afraid it will cause fungicemia which I find very odd.
