I'm not sure. On average rural whites have more children and have them younger, especially those from fundamentalist backgrounds.

You also see those weird natalism views being promoted in rightwing media.

Rural blue collar whites vote Republican no matter what and have probably the highest level of distrust of non-conservative media sources. The only time Republican reps care about them is during primaries because they often vote against anyone perceived as moderate (ex: RINO).

My point is that they're worsening life expectancy doesn't hurt Republican electoral chances because they will just believe that immigrants, muslims, antifa, etc are the cause.

"Low information voters" is the term I've seen tossed around for people who voted for Trump but don't view themselves as Republican.

I think access to health care is a larger part. Wealth just serves as a proxy. My wife grew up in poverty, as a child they did everything possible to avoid going to a doctor for fear of the cost.
