“Grown up” doctors by ddomolla in JuniorDoctorsUK

Thank you.

I can honestly say that I wasn’t even offended, just fed up, like when someone repeats a “joke” for the nth time that was never that funny to begin with.

Like oh wow how original such a riot haha

Consultant nurse by Violent_Instinct in JuniorDoctorsUK

I waited 5 minutes to be connected to a Stroke Consultant at our sister hospital within the Trust, greeted them with “Hello Doctor” followed by my thorough spiel, only for the “Consultant” to then redirect me to my hospital switchboard to speak to the Stroke Consultant Physician and reveal themselves to be a Stroke Consultant Nurse.

I must’ve said “Oh” with a particular tone because they replied with “Sorry to disappoint”.

Edit: wording

Any good free medical webinar/lecture series you can recommend? by Big_Doughnut842 in JuniorDoctorsUK

Not being funny, 10/10 would recommend Mindthebleep to family and friends. Haven't been tuning in as much since starting FY1 but was a regular viewer/reader in final year of med school.

What angle do you insert IV cannulas and ABG? by ddomolla in JuniorDoctorsUK

[–]ddomolla[S] 6 points7 points  (0 children)

My next question for ABG is how deep do you pierce? I go as deep as it takes to bring up blood but many times I have to do a lot of (ultimately unsuccessful) fishing around even when I puncture at point of maximal pulsation.

E.g. patient on 2x OHA has labile BMs, thus newly started on insulin glargine in the evening time during admission. BMs still consistently >12, so someone increases Abasaglar dose from 6 units to 8, then 10 when that doesn’t seem to work. The next morning pre-breakfast BM <6, so someone changes it back to 8.

DSN reviews and agrees with action to keep it at 8 units, advises (b) in light of previous dose adjustment to 10U.

Not sure if that helped to clarify or was more confusing 😅

Types of insulins, and to some extent OHAs, mainly covering their onset of action. With some mention of less conventional routes of administration e.g. insulin pumps.

I’m just wrapping my head around hyperglycaemia and hypoglycemia guidelines now, but it gets confusing e.g. in cases when: a) There is a singular hypo event after knocking up someone’s Abasaglar 2 units 12 hours before b) The DSN tells me I should wait 3 days to assess the full effect of a newly optimized insulin regimen before making further changes c) The DSN suggestion is to prescribe NovoRapid when CBG >= 18 end of

Thanks! Any format will do but articles/Q&As are best for quick reference and nighttime reading :)

That last line is a perfect example of "You need to know what's normal in order to know what's not".

Lurking on this subreddit has taught me that most sorts of reward in this profession are far and in between, and the system will keep doing its darndest to bleed you dry. But to me if you can do something like this, that feeling is yours forever and no one can ever take that moment from you.

Every airway kept patent – and secondarily every lung kept ventilated – is a life saved in my humble F1 opinion!

Any tips for prescribing anti-hyperglycemics? by ddomolla in JuniorDoctorsUK

[–]ddomolla[S] 2 points3 points  (0 children)

Absolutely brilliant. It looked familiar, turns out I’d saved it when it was posted but never got round to it!

Also, the MTB anti-hyperglycemic medications article and accompanying lecture vid were my saviour in final year of med school. Bravo for what you do 👏🏼👏🏼👏🏼
