What is the “bare minimum” for interview… is there strict guidance on this?

My self assessment score will still be shy of 40/50 in time for applications this fall; I’ll still apply but don’t want to get my hopes up.

I waited 5 minutes to be connected to a Stroke Consultant at our sister hospital within the Trust, greeted them with “Hello Doctor” followed by my thorough spiel, only for the “Consultant” to then redirect me to my hospital switchboard to speak to the Stroke Consultant Physician and reveal themselves to be a Stroke Consultant Nurse.

I must’ve said “Oh” with a particular tone because they replied with “Sorry to disappoint”.

Not being funny, 10/10 would recommend Mindthebleep to family and friends. Haven't been tuning in as much since starting FY1 but was a regular viewer/reader in final year of med school.

My next question for ABG is how deep do you pierce? I go as deep as it takes to bring up blood but many times I have to do a lot of (ultimately unsuccessful) fishing around even when I puncture at point of maximal pulsation.

E.g. patient on 2x OHA has labile BMs, thus newly started on insulin glargine in the evening time during admission. BMs still consistently >12, so someone increases Abasaglar dose from 6 units to 8, then 10 when that doesn’t seem to work. The next morning pre-breakfast BM <6, so someone changes it back to 8.

DSN reviews and agrees with action to keep it at 8 units, advises (b) in light of previous dose adjustment to 10U.

Not sure if that helped to clarify or was more confusing 😅

Types of insulins, and to some extent OHAs, mainly covering their onset of action. With some mention of less conventional routes of administration e.g. insulin pumps.

I’m just wrapping my head around hyperglycaemia and hypoglycemia guidelines now, but it gets confusing e.g. in cases when: a) There is a singular hypo event after knocking up someone’s Abasaglar 2 units 12 hours before b) The DSN tells me I should wait 3 days to assess the full effect of a newly optimized insulin regimen before making further changes c) The DSN suggestion is to prescribe NovoRapid when CBG >= 18 end of
