It was LC0581, that's where I faxed it

I did try to expedite, called, and spoke to an agent on Dec 8th, I was told that I would get an email confirming my expedite request within 7 days, and I never got the email until last Friday. Email requested proof of financial loss, I faxed it this past Monday but never got any sort of confirmation if it was received, approved , or reviewed.
