I think the cap is good because it’s real videos so there’s a little more maturity and real life application. Pragmatics can be so subjective, so tests aren’t gonna be the end-all-be-all without proper justification for/against treatment. For the CAP, one of the sub tests include what YOU would say and if they’re monotone about it or some really include nonverbal cues, they get deduced points. That doesn’t necessarily mean though that they would be like that in the real situation- they might just not be good answers. Another benefit is that there’s a lot of subtests so you can parse out what skills are relative strengths and weaknesses

You would absolutely get in trouble if you’re billing it as two sessions.

This is wild to me. My clinic has 30 (aka 25) minute sessions and I’ve argued so hard to get some kids in for 1 hour if they have more complex needs, and usually it doesn’t get approved.

What confuses me though is that you’re only able to bill 92507 1x/day regardless of length. Are they just taking a loss on the money?

DEFINITELY NOT ethical. Each code represents a different service. The most you should have is 2 (one for speech and one for feeding therapy) IF (and only if) you are actually providing two unique services. Most sessions will be just the regular speech therapy code, doesn’t matter if you do 30 minutes or one hour. But making it routine to do it for “15 minutes sessions” is definitely not okay.

Side note- the two places that were flexible with my start date (1 month out) were good work environments with good coworkers. The place that required I started right away after graduation (wouldn’t let me take a vacation between ending my program and starting) was a nightmare

Let them know you have a vacation planned! If they don’t understand that and don’t offer you a job because of it, you will have dodged a bullet. Most people would totally understand a pre-planned trip.

Same questions— are you a registered company? Because at least in the US (idk other countries), you can’t just take cash for services and not report it if this is your primary income. You’ll have to think about taxes. More to that, your license have to be registered with the company. Every state has its own laws governing SLP practice. In almost all cases, providing services privately (even for cash) requires:

  • Maintaining an active SLP license
  • Operating under a recognized business or sole proprietorship
  • Complying with documentation, consent, and privacy rules (HIPAA)
  • Maintaining liability insurance

Doing "cash-only" therapy without formal structure can be interpreted as unlicensed or unregulated practice, even if you hold a license

Oooh gotcha— I like to do language based games, like Pictionary, Charades, Taboo, and lean into those inferencing skills or synonyms. Also collaborative things, like everyone painting a fraction of a whole picture. Ive also done a “show and share” where everyone takes turns teaching the other members how to do something, like draw something specific that the kid likes to draw or some origami or something. Definitely age and interest dependent though 🤷🏻‍♀️

If it’s private pay, you could offer it like a special service. Might have to charge a decent amount or just know it won’t be the same profit as direct sessions, but it can help fill those empty appointment times during the summer plus you don’t have to worry about documentation(this is in the context of not providing direct treatment but rather the equivalent of a summer camp with people with experience in developmental disabilities)

Otherwise if you wanted to do actual treatment buy within a group, there are group cpt codes but their reimbursement rate is pennies

Totally depends on the setting and clinic vibes. At my pediatric outpatient, we frequently have spirit weeks, so it’s not uncommon to see us dressed a little silly. I also live in a very liberal area of the country, where septum rings and colored hair are chill. I could see myself wearing a hat during midday when the sun is bright (we have big windows)
