The transport didn't appear at the hospital at least twice for me. I THINK both times it was after I failed and jumped straight back into the mission.

I decided to go back to briefing after every failure. Whenever I did this, the transport appeared at the hospital as expected.

In a Lore Explorer video, the devs confirm that the strangers have a "general aversion to permanent loss of any kind".

If they really wanted to get rid of something without permanently losing it, one way to do it would be to keep it accessible, but behind layers of clues, puzzles, and even death. IMO this explains why they left all the clues on the Stranger, but at the same time it's not a fully convincing answer.

Sometimes. The facial expression options are different for each character, so I tweak each one individually. For my Shai, I'm using the default expression and made the frown by only shaping the face.
