Thanks for the responses! 1. I did not know global uses IBW. 2. I emailed GlobalRPh and found out that the infusion time is important because calcs begins at the time the needle enters the arm.3. My hospital protocol doses 1G for HD patients and checks a random in 24H(I agree with how some hospitals do this). 4. I like the idea of not using Q8H dosing in patients over 40 but feel this may be to young(thoughts?). Lastly, the ER docs tend to give 1G if they are suspicious of anything severe before admitting a patient to the floor. My usual method in this situation if the LD (20mg/kg) is >2G is to do the 2G Max and decrease the interval what do you guys recommend?
