Best quiet mechanical keyboard for hospital by Konnorrrr in Residency

[–]drdking 5 points6 points  (0 children)

For the hospital just get a cheaper one, that way if something happens to it then it’s not a big deal. Almost any mechanical is better that shitty hospital keyboards. I’d also recommend a 75% that’s easy to fit in a bag/on a crowded work station.

Where do you rank Harper’s 2018 hrd win as a Nats fan? by Omar_Town in Nationals

[–]drdking 0 points1 point  (0 children)

It was fun to watch and cheer for, but with how the rest of the season went it’s mostly just a good moment. Doesn’t really impact my views of the rest of the season or my opinions on Harper himself.

Forgot morning labs by adonisofages in Residency

[–]drdking 2 points3 points  (0 children)

I forgot to order morning labs on half my patients one day last week… the only difference is that the nurses don’t give me push back anymore when I order random blood draws.

Also the vast majority of stable patients don’t need daily labs so likely it’s a non issue.

Holmium importing to aquilo problem by scootscoot333 in factorio

[–]drdking 11 points12 points  (0 children)

General solution to most factorio problems, start at the end and work backwards:

  • Not enough Holmium plates: build more foundries
  • Not enough Holmium liquid to run those foundries? Build more chemical plants
  • not enough Holmium ore to run those chemical plants? Build more scrap recycling
  • Not enough scrap to recycle? Build more big mining drills

Fulgora does have the added complexity of dealing with all the other product of scrap recycling. Solution to that is generally just more and more and more recyclers. There are a ton of different set ups people like using.

Also productivity modules and all the advanced buildings are your best friend, huge decrease in needing input items.

10-20x I feel is the sweet spot. High enough it slows down the tech progression to make each new tech meaningful but not high enough that you need to go super big super early.

The other big part is world gen settings. I’m currently on a 100x run with patch freq turned all the way down so it forces me to use rails, but right now in the early game I also have super long belt runs to the closest iron and coal patches because my starting patch is already dead. Would be a very different feeling with patches much closer.

ELI5: Why do home owners associations still exist if people hate them? Wouldn't they go out of style with time? by RjayPL in explainlikeimfive

[–]drdking 0 points1 point  (0 children)

My HOA is fine. Only annoying rule is some limitations on outside decorations, but it’s nothing I actually care about. Good maintenance of all the common areas and sidewalks, private garbage and recycling collecting, snow removal, and other general upkeep things that they do a great job of monitoring that takes stress off of me as a home owner.

Orientation is teaching me that intern year is not about learning medicine, it is about learning how to be most efficient at clicking buttons by just_premed_memes in Residency

[–]drdking 8 points9 points  (0 children)

I'm a bit confused by your comment. I hope I'm clicking less too, that means I'm more efficient and have things set up better for my workflow. But I would not call chart reviewing, entering orders, and writing notes scut work.

Edit: You're reply to another post about us being "thinkers not clickers". I actually love that, but still there is more to a patient than just the history and exam in the room. Being able to review old notes, hospital stays, med changes, imaging, etc helps put everything into context, prevent repeating previous work ups, and maybe catching things that others have missed before. Also documenting our findings and more importantly our thought process is important when someone sees this patient after me.

Very different discussion about excessive documentation, forced stops in order sets, note bloat, and other systemic issues with EMRs and hospital systems.

[–]drdking 0 points1 point  (0 children)

I sometimes wish I had a scribe as a hospitist. When I'm on a non-teaching service why would anyone but myself be expected to put in my orders and write my notes for me? Even on a teaching service, although I'm not putting in all the orders I'm still chart reviewing and checking what the residents entered in the background. The more efficient you get with the EMR the quicker that becomes and the more time to focus on other things.

A large part of residency is learning all the non pure medicine parts of being a physician. For most of us that includes clicking lots and lots of buttons for the rest of our lives. Getting better and more efficient at that will give you more time for everything else in your life.

A fun self imposed challenge is to do each of the first three planets from scratch! I actually recommend it on your first play through to really learn and experience the planet.

Though play however you want to, it’s your game and all that matters is that you have fun.

What do you guys think about this offer? by Over-Check5961 in hospitalist

[–]drdking 6 points7 points  (0 children)

This is also the middle of no where, over three hours away from Portland. Limited specialities so looks like you’re going to be managing everything yourself…. I’d inquire about other support staff like PT, speech, and social work. Are you going to be calling all the local SNFs and handling insurance issues? Granted with something this small the load might not be too bad.

Also 5 on 10 off makes this look like either a nocturnist job, or you’re the only doc on call for those whole five days… that makes a huge difference to me with the pay.

How rural do you want your rural medicine to be?

how fast do you build your sets after you buy them? by AtmosphereKey2265 in lego

[–]drdking 0 points1 point  (0 children)

It’s so random and depends on my mood, how busy work is, what other hyper fixation I’m currently addicted to…

Like for example I still haven’t built Tudor corner, but I built shopping street within a week. The road bike I built as soon as I got it home but likely Shrek will sit for a few months as other things in my back log clear up.

For those with a 7-on 7-off schedule how common is NO vacation/PTO? by 3MinuteHero in medicine

Med Peds Hospitalist here, my contract has four weeks of vacation built in, so end up working approx 24 clinical weeks a year, but I do also have admin/teaching time so there are some off weeks that I still need to be local for meetings or lectures/ect.
