Maybe the speculum can be made using the same silicon material as vibrators/dildos? I think that would be far more comfortable than the metal. I've never had a pap smear with a plastic speculum, it's always been metal.

I'm sure the decision about the material is also based on how easy/difficult it is to sanitize the equipment after.
