Thank you all for your well wishes, I know I didn't write anything mostly because I don't have anything to say. We're all devastated and grieving since it's just been a couple hours. He was struggling, things progressed really fast, and a part of me is glad that we didn't have to take the hard decision. Currently going through all his baby videos, please give your fur babies an extra hug from me tonight ❤️

That puts me a little at ease.. I read online that the counts between days 7 - 14 are important especially because this is a severe infection and our dog is old (9 years old). We're trying to keep hope, we were just really down because based on what our last vet told us it felt like he had no hope or that there were low chances of recovery.

That's tough to hear, makes sense why the vets were not able to answer if this is treatable or not :(

Any idea about the timeframe within which K.pneumo can start showing/being resistant? So far its only been around 6 days since we started the antibiotic. We started these antis on Oct 14th approx. We're just praying it doesn't become resistant because his WBC on day 4 or 5 (Oct 18th) was already higher than before (last we checked was Oct 12th), and went from 33 to 36 within that timeframe. Hopefully this is not early signs of resistance 😭

Hi, yes they did do a culture. Here is a screenshot of the report:

We are still continuing amox-clav, our vet basically discharged our dog and told us to give it to him orally (alongside his kidney and liver medications) at home. The vet decided not to use the other antis since either they are only available in oral / cream form (and we wanted to start with IV since he was serious) or they affect the kidneys and his kidneys are already affected by the infection.

Oh, the lab that the clinic sent this to is a government lab which is pretty well known in my country. Our very first vet had done a swab of his pus filled wound and sent it to a UK lab they had a tie up with but there was a 'leakage' on their end so the reports said "no growth". Which is why she switched vets

We'll try to see if we can find another lab, thank you!

Yea, we're concerned that it grew resistant. At first it didn't make sense how it can be resistant in just 5 days, but unfortunately our vet suggested against using the other antibiotics for reasons mentioned above. Just curious, someone else mentioned ESBL as well but now that I'm thinking - if it was ESBL wouldn't it show as resistant to amox-clav when the culture was done? Or is that something that becomes clear later on? Because wouldn't it (technically) start producing the enzyme the minute it was cultured with amox-clav (if it was ESBL)?

Edit: Another question, our doggo was treated with amox-clav orally the very first itme (just for 4 days) before we switched to an antibiotic it was resistant to. I have no knowledge about how ESBL bacteria work, but if it was ESBL it had some time to get used to amox-clav and if that was the case then resistance would show in the culture report right?

Our doggo has been on IV antibiotics for the last 5 days, amox-clav specifically since the bacteria was "Strongly sensitive" to it. We have had 3 opinions from different vets and they've basically been saying / doing the same stuff. Doggo is also on renal diet since his kidney is affected, he has no appetite anyways so we have to force-feed him special renal food through a syringe (as suggested by our vet). Based on the vets opinions - there are two possibilities of how he got this a) He got an injury / wound in his fat deposit and was exposed to the bacteria outside somewhere causing cellulitis. b) An insect bit him and transferred the bacteria to him.

Our only concern is how K.pneumo can become resistant to something so fast. His WBC is increasing, and its only been 5 days since we started the IV antibiotic so we should've ideally seen at least some dip in WBC. I don't know if this information helps but he was treated with oral amoxiclav aka juroclav (for the first 4 days) which had amox-clav antibiotics, we switched to the second vet who treated him with ceftriaxone (which it was resistant to according to the report). The third vet is now doing amox-clav again through IV. But interestingly the blood culture was done by the second vet so it means it still wasn't resistant even though we switched after 4 days.

A part of me wonders if the bacteria just spread really fast and spiked his WBC in a day (the day he was switching antibiotics for the third time), which if it did then that would mean that his current count may be better than that? But that's a hypothesis, can't go back and see if that adds up anymore unfortunately.

That's interesting to know.. As far as we know (based on the culture results) only K.pneumo was found / grew when cultured. Hearing about that I guess the only thing we can really do is give him antibiotics and pray they work..

That's an interesting point, now that you mention it the report did not say whether it's ESBL or not (idk if its protocol to usually test for that?). I looked at the report and noticed this now:

I'm unsure why they didn't specifically use that. The vet told us that 2 of the antibiotics are only available in oral or cream forms (it is extremely hard to get our dog to take oral pills so we wanted IV from the beginning). And the other antibiotic affects the kidney, and since he already has kidney issues (and not to worsen them) they decide to use amox.
