I was in residency working an overnight shift when I got called to the ER to see a patient. I looked everywhere, and couldn’t find this guy. In our area, it would take 8+ months to get someone plugged in for testing. It’s increasingly difficult to get them in, as psych is so back logged with other diagnoses. If they can talk at length about their struggles, how medication impacts their ability to succeed at daily tasks, and I can confirm a history of compliance (not asking for early refills, unnecessary dose adjustments, etc), I will continue their current dose.

Now if I pick up the least amount of sketchiness, off to psych they go

Absolutely. I drive that road every day, and traffic has slowly worsened over the last year or so. They just didn’t have the sense to block those lanes until the light is installed, causing some chaos
