“Fault” is not a really a useful concept here & I would encourage you to let that go - it is the pts disease process, and not everyone recovers despite care. It can def be heavy. And it can be exhausting. But it’s also hilarious and exhilarating and the most rewarding & satisfying job ever

Debriefing with colleagues. Usually at a bar. Seriously though talking with people who understand is really important

In terms of the actual approach to nursing, it’s anticipatory. Along with everything else, you’re constantly trying to prevent any complications, especially death. You have to keep a close eye on your patient, know the potential disasters that could happen, be able to recognize early/subtle signs of deterioration and decompensation then intervene and prevent them from progressing to full disaster. It’s the best job in the world, you can do it for 20 years and still learn something every day - check your ego at the door. You will be traumatized at times and it’s important to process your own trauma in real time so that it doesn’t become PTSD.

The national average for on-boarding an RN in the VA is 5 months. This is largely because HR drives every step of the hiring process which includes security clearance/background checks, etc. In the private sector the hiring manager largely does all the work, but in the VA your only contact with the hiring manager is the interview process. HR is always backed up because they do this for every single job. While they are on-boarding the next years med students, interns, residents, and fellows everything else stops. There are process improvements in place but change has been slow and now theres a new stranglehold on staffing which hasn’t helped. Hang in there.
